摘　　要：视野损伤是垂体腺瘤(Pituitary Adenoma,PA)术后常见并发症,主要由于PA不断浸润性生长压迫视交叉,或术中压迫损伤视神经,导致视神经损伤。本研究从“精血同源”理论出发,结合手术的中医化属性、PA术后视野缺损特征及其临床表现等,探讨PA术后视野缺损的中医病因病机及治疗。PA术后视野缺损的病因为外伤(或“外邪”),病位在肝与肾,病机核心在于外伤(或“外邪”)损伤精血,导致肝肾阴虚,可兼见气虚血瘀。本研究根据PA术后视野缺损临床特点,从精血同源角度将其分为急性期和缓解期,急性期多以气虚血瘀为主,缓解期多以肝肾阴虚为主,探讨PA术后视野缺损治疗的中医认识,为临床应用提供参考。Visual field injury is a common postoperative complication of pituitary adenoma,mainly due to the continuous Vinvasive growth of pituitary adenoma compressing the optic chiasma,or intraoperative compression and injury of the optic nerve,resulting in optic nerve injury.Based on the theory of homology of essence and blood,this paper discusses the TCM etiology,pathogenesis and treatment of postoperative visual field defect of pituitary tumor,combining the TCM attribute of surgery,characteristics and clinical manifestations.The disease of visual field defect after pituitary tumor surgery is due to trauma or external evil-Qi(邪气),the disease is located in the liver and kidney,and the core of the pathogenesis is trauma or external evil-Qi damage to the blood,resulting in liver and kidney Yin(阴)deficiency,which is accompanied by Qi deficiency and blood stasis.According to the clinical characteristics of postoperative visual field defect of pituitary tumor,it is divided into acute stage and remission stage from the point of the same origin of essence and blood.This article discusses the understanding of traditional Chinese medicine in the treatment of visual field defect after pituitary tumor surgery,in order to provide reference for clinical application.
