王艳锋教授针药联合关节松动术治疗交感型颈椎病临证经验

摘　　要：交感神经型颈椎病(Sympathetic Cervical Spondylosis,SCS)是由于颈椎间盘退行性变、椎间关节退变及其继发的相应病理改变刺激颈部交感神经纤维,导致交感神经功能异常的综合征。该病发病机制复杂,症状表现多样且易反复发作。中医在SCS的治疗上积累了丰富的经验,具有独特优势。王艳锋教授将本病归属于“项痹”范畴,认为SCS证属本虚标实,治疗时主张“针药结合、筋骨并重”,提出了针药联合动态关节松动术的治疗方案。针刺疗法选取督脉、手阳明大肠经、足少阳胆经腧穴电针以促进颈肩部血液循环,理“筋”止痛以治“标”,中药治疗当以“补益肝肾、滋阴养血”为法以治“本”,配合Mulligan动态关节松动术纠正颈椎失稳状态以正“骨”。此套疗法针对SCS进行综合治疗,可有效缓解患者症状,延缓疾病复发。本研究结合案例介绍王艳锋教授应用针药联合动态关节松动术治疗SCS的临床经验,对其遣方用药及中医调理特色予以总结。Sympathetic cervical spondylosis(SCS)is a syndrome of abnormal sympathetic nerve function due to the stimulation Sof cervical sympathetic nerve fibers by degenerative intervertebral discs,intervertebral joint degeneration,and their secondary pathological changes.The pathogenesis of this disease is complex,the symptoms are diverse and easy to recur.Traditional Chinese medicine has accumulated a wealth of experience in the treatment of sympathetic cervical spondylosis,occupying a unique advantage.Professor WANG Yanfeng classifies this disease into the category of Xiangbi(项痹),and believes that the syndrome of sympathetic cervical spondylosis is difficiency in origin and excess in superficiality.The professor advocates combination of acupuncture and medicine,paying equal attention to muscles and bones,and proposes the treatment therapy of acupuncture and medicine combined with joint mobilization.This therapy selects the acupoints of Du meridian(督脉),hand Yangming(阳明)large intestine meridian,foot Shaoyang(少阳)gallbladder meridian to promote blood circulation in neck and shoulder,regulate“tendons”and relieve pain to treat“symptoms”.Traditional Chinese medicine treatment takes“tonifying liver and kidney,nourishing Yin(阴)and blood”as the method to treat the disease from root cause.Mulligan dynamic joint mobilization is used to correct the instability of cervical spine and correct the bone.This set of therapies can effectively relieve the symptoms of patients and delay the disease recurrence.This paper introduces Professor WANG Yanfeng’s clinical experience in the treatment of sympathetic cervical spondylosis by acupuncture and medicine with dynamic joint mobilization through one case,and summarizes the prescriptions and traditional Chinese medicine conditioning characteristics.
