摘　　要：“五脏六腑皆令人咳,非独肺也”,由于小儿脏腑娇嫩,行气未充,加之复杂的病因使咳嗽诊治难度加大,临床上常出现咳嗽久治难愈的现象。小儿久咳常因肺脾不足,素体本虚,复感风寒邪气,因正虚抗邪无力,致使风邪恋肺,造成卫气疏泄失常,出现营卫失和。因此在治疗中除补益肺脾、调和营卫,还应兼顾宣肺疏风、化痰止咳。中医药治疗咳嗽历史悠久且疗效确切,桂枝汤能补脾益肺、调和营卫,止嗽散能宣肺疏风、化痰止咳。临床中因小儿肺脾不足,风邪恋肺,伴见营卫不和而久咳者,可予两方加减合用,使营卫自和,肺脾功能恢复。“According to traditional Chinese medical theory,all organs can cause cough,not only the lungs”.Due to the delicate nature of children's organs and the incomplete flow of Qi,combined with complex etiologies,the diagnosis and treatment of cough are challenging.Persistent cough often occurs in clinical practice.This is often attributed to insufficient lung and spleen function,as well as a weak constitution that is further compromised by wind-cold invasion.The body's defense mechanisms are unable to effectively combat the external pathogenic factors,leading to wind-pathogen retention in the lungs and resulting in the disorder of defensive Qi and nutritional Qi.Therefore,in treatment,apart from nourishing the lungs and spleen and regulating the balance of defensive and nutritional Qi,it is also necessary to consider clearing the lungs and promoting wind dispersion,as well as resolving phlegm to alleviate cough.Chinese medicine has a long history of effectively treating cough.Guizhi decoction can nourish the spleen and lungs,regulate defensive and nutritional Qi,while Zhisou powder can clear the lungs and promote wind dispersion,resolve phlegm and alleviate cough.In clinical practice,for children with persistent cough due to insufficient lung and spleen function,wind-pathogen retention in the lungs,and imbalance of defensive and nutritional Qi,a combination of these two decoctions can be adjusted according to individual conditions to achieve harmony between defensive and nutritional Qi,as well as restore lung and spleen function.
