从痰瘀论治冠心病伴失眠:基于“津血同源”理论

摘　　要：中医认为冠心病伴失眠的病因主要为饮食不节、情志不畅、劳逸失调、年老体虚,病机为瘀血、痰浊等病理产物瘀滞于心脉,致使心主血脉功能失常,出现心前区疼痛、憋闷不适等症状,瘀阻心脉,使血不养神,心神失养继而出现失眠。痰浊与瘀血二者为人体津血代谢失常所产生的病理产物,而血液和津液皆为水谷精微所化,二者可相互资生、相互转化,津血同源,痰瘀二者亦是如此。因此基于“津血同源”理论,从“痰瘀”角度论治冠心病伴失眠,有助于为治疗冠心病伴失眠患者心前区疼痛、憋闷、睡眠缺失等症状寻找新的治疗思路。Traditional Chinese Medicine believes that the main causes of coronary heart disease accompanied by insomnia are irregular diet,emotional disorders,imbalance between work and rest,and old age and physical weakness.The pathogenesis is the stagnation of pathological products such as blood stasis and phlegm in the heart meridian,leading to dysfunction of the heart's main blood vessels and symptoms such as pain and discomfort in the precordial area.Blood stasis obstructs the heart meridian,causing the blood to not nourish the mind,and the mind to lose its nourishment,resulting in insomnia.Phlegm turbidity and blood stasis are pathological products produced by the abnormal metabolism of body fluids,and both blood and body fluids are transformed by the essence of water and grains.The two can be mutually produced and transformed,and the same goes for fluid and blood,as well as phlegm and blood stasis.Therefore,based on the theory of“same origin of fluid and blood”,discussing the treatment of coronary heart disease with insomnia from the perspective of“phlegm and blood stasis”can help find new treatment ideas for symptoms such as precordial pain,suffocation,and sleep loss in patients with coronary heart disease with insomnia.
