基于玄府-络脉理论探析脑小血管病

摘　　要：玄府、络脉源自《黄帝内经》,是中医学从微观结构认识人体的体现,均具有分布广泛、结构微小的特点。二者在功能上相互依存,以通为用,共同保证机体流通气液、渗灌气血、沟通营卫、调和阴阳、运转神机等作用正常实现;病理上相互影响,玄府开阖失司、络脉荣气虚滞,临床上常出现玄闭络瘀、络阻玄郁等玄病及络、络病及玄、玄络共病的病证。脑小血管病是多种危险因素影响脑内小动脉、微动脉、毛细血管、微小静脉所导致的一系列临床、影像和病理综合征。脑小血管在形态和功能上与玄府、络脉高度一致,基于玄府-络脉理论,认为络损玄病、髓减脑消、神机失用为脑小血管病的病理机制,调畅玄府、通补络脉、填精益髓为治疗大法。The Xuanfu and collaterals originate from the Huangdi Neijing,which are manifestations of traditional Chinese medicine's understanding of the human body from the microscopic structure.They both have the characteristics of wide distribution and small structure.The two are functionally interdependent and work together to ensure the normal realization of the body's functions such as the circulation of Qi and liquid,the penetration of Qi and blood,the communication of nutrient Qi and defensive Qi,the reconciliation of Yin and Yang,and the transportation of spiritual mechanism.Pathologically,they influence each other.If the Xuanfu opening and closing function is abnormal and/or the collaterals are deficient and stagnant,it will lead to the clinical co-morbidity of Xuanfu and collaterals.Cerebral small vessel disease is a series of clinical,imaging and pathological syndromes caused by multiple risk factors affecting the small arteries,arterioles,capillaries and venules in the brain.The morphology and function of cerebral small blood vessels are highly consistent with Xuanfu and collaterals.Based on the theory of Xuanfu-collaterals,it is believed that damage to collaterals,loss of marrow and brain,and apraxia of the mind are the pathological mechanisms of cerebral small vessel disease.Regulating Xuanfu,unblocking and Nourishing collaterals,and replenishing essence and marrow are the main treatments.
