补肾调经方联合脱氢表雄酮治疗卵巢储备功能减退临床研究

摘　　要：目的:探究自拟补肾调经方联合脱氢表雄酮(DHEA)治疗卵巢储备功能减退(DOR)的临床疗效。方法:选择74例观察期未进行激素补充治疗(HRT)的DOR患者,其中试验组40例予以补肾调经方联合DHEA治疗;对照组34例予以DHEA治疗,疗程均为3个月经周期,比较两组患者的性激素(FSH、LH、E2、T)、抗缪勒管激素(AMH)、卵巢体积、基础窦卵泡(AFC)、改良Kupperman量表总积分及中医症状评分的变化。结果:试验组治疗后FSH明显低于治疗前及同期对照组(P<0.05),而LH、E2、T及AMH在两组间差异无统计学意义(P>0.05);试验组治疗后AFC明显高于治疗前及同期对照组(P<0.05),而卵巢体积两组间差异无统计学意义(P>0.05);试验组治疗后改良Kupperman量表总积分明显低于治疗前及同期对照组(P<0.05);试验组治疗后部分中医症状评分明显低于治疗前及同期对照组(P<0.05)。结论:补肾调经方联合DHEA治疗DOR可获得满意疗效,明显优于单独DHEA治疗。Objective:To observe the clinical efficacy of Bushen Tiaojing formula combined with dehydroepiandrosterone(DHEA)on the treatment of diminished ovarian reservation patients.Methods:A retrospective analysis was used,a total of 74 patients with diminished ovarian reservation(DOR)who refused hormone replacement therapy(HRT)were divided into two groups.The experimental group with 40 cases was treated with Bushen Tiaojing formula combined with DHEA,the control group with 34 cases was treated with DHEA,both groups were treated for 3 menstrual cycle.The hormone levels(FSH、LH、E2、T),the anti-Müllerian hormone(AMH),the changes of ovarian volume,the antral follicle counting(AFC),the improved Kupperman total score and the traditional Chinese medicine symptoms were compared in the two groups.Results:After treatment,the levels of FSH in experimental group were significantly lower than before treatment and the same period in the control group(P<0.05),the levels of LH,E2,T and AMH had no obvious difference between the two groups(P>0.05);the AFC in experimental group were significantly higher than before treatment and the same period in the control group(P<0.05),the changes of ovarian volume had no obvious difference between the two groups(P>0.05);the improved Kupperman total score in experimental group was significantly lower than the treatment before and the same period in the control group(P<0.05);the part of traditional Chinese medicine symptoms score in experimental group was significantly lower than the treatment before and the same period in the control group(P<0.05).Conclusion:Bushen Tiaojing formula combined with DHEA achieved remarkable curative effect in the treatment of DOR,which was better than DHEA therapy alone.
