摘　　要：目的:探究项七针联合穴位推拿治疗气滞血瘀型颈性眩晕患者的疗效及对椎动脉血流动力学与血清神经肽Y(NPY)、尾加压素Ⅱ(UⅡ)水平的影响。方法:选取气滞血瘀型颈性眩晕患者120例,按数字随机表法分为对照组和研究组各60例。对照组患者给予常规西药治疗,研究组采用项七针联合穴位推拿治疗。比较两组患者治疗前后的中医证候积分、颈性眩晕症状与功能评估量表(ESCV)评分、血流动力学指标及血清NPY、UⅡ水平,并于治疗后评估两组的临床疗效及不良反应发生情况。结果:治疗后,两组患者中医证候积分、PI及血清NPY、UⅡ水平均低于治疗前,Vm、ESCV评分均高于治疗前(均P<0.05),且研究组的改善程度均优于对照组(均P<0.05);研究组的临床总有效率明显高于对照组(P<0.05),两组治疗中不良反应总发生率差异无统计学意义(均P>0.05)。结论:项七针联合穴位推拿治疗气滞血瘀型颈性眩晕疗效显著,可明显改善患者椎动脉血流动力学,减轻临床症状,且具有较高的安全性。Objective:To investigate the therapeutic effect of nape seven needles combined with acupoint massage on cervical vertigo patients with Qi Stagnation and Blood Stasis and its impact on vertebral artery hemodynamics and serum levels of neuropeptide Y(NPY)and urotensinⅡ(UⅡ).Methods:A total of 120 patients with cervical vertigo with Qi Stagnation and Blood stasis were prospectively selected and randomly divided into a control group and a study group,with 60 patients in each group,using a digital random table method.The control group of patients received conventional Western medicine treatment,while the study group received treatment with nape seven needles combined with acupoint massage.Compare the traditional Chinese medicine syndrome scores,cervical vertigo symptoms and functional evaluation scale(ESCV)scores,hemodynamic indicators,and serum NPY and UⅡlevels of two groups of patients before and after treatment,and evaluate the clinical efficacy and incidence of adverse reactions of the two groups after treatment.Results:After treatment,the TCM syndrome score,PI,serum NPY,and UⅡlevels of both groups of patients were lower than before treatment,and the Vm and ESCV scores were higher than before treatment(all P<0.05).Moreover,the improvement degree of the study group was better than that of the control group(all P<0.05);the total clinical effective rate of the research group was significantly higher than that of the control group(P<0.05),and there was no significant difference in the total incidence of adverse reactions between the two treatment groups(both P>0.05).Conclusion:The combination of nape seven needles and acupoint massage has a significant therapeutic effect on cervical vertigo of Qi Stagnation and Blood Stasis,which can significantly improve the hemodynamics of the vertebral artery in patients,alleviate clinical symptoms,and has high safety.
