针灸治疗神经根型颈椎病根性疼痛机制研究进展

摘　　要：近年来颈椎病的发病率逐渐升高,主要以神经根型颈椎病(CSR)最为常见,研究表明针灸治疗CSR取得良好的疗效,但其作用机制不明确,本文总结了近年来针灸治疗CSR根性疼痛的机制研究文献,结果显示,针灸治疗CSR是多机制、多层次的作用结果,主要采用普通针刺、电针、温和灸、麦粒灸等方法,本文从调节外周敏化、调节中枢敏化和调节神经细胞的自我保护方面对针灸治疗CSR相关研究进行总结,其中包含抑制外周炎性因子的表达、抑制中枢炎症因子及相关致炎通路的表达、调节神经胶质细胞活化,调节谷氨酸信号传递及中枢神经递质相关信号通路、调节突触结构的可塑性、调节细胞自噬凋亡及相关信号通路、调节外周氧化等方面,以此来探究针灸治疗CSR的机制,以期为临床治疗CSR提供一定的参考。In recent years,the incidence of cervical spondylosis has gradually increased,and cervical spondylotic radiculopathy(CSR)is the most common.Studies have shown that acupuncture has a good effect on CSR,but its mechanism is not clear.This paper summarizes the literature on the mechanism of acupuncture in the treatment of CSR root pain in recent years.The results show that acupuncture treatment of CSR is the result of multi-mechanism and multi-level action.This paper summarizes the related studies on acupuncture treatment of CSR from the aspects of regulating peripheral sensitization,central sensitization and self-protection of nerve cells,including inhibiting the expression of peripheral inflammatory factors,inhibiting the expression of central inflammatory factors and related inflammatory pathways,and regulating the activation of glial cells.Regulate glutamate signal transduction and central neurotransmitter related signal pathways,regulate synaptic plasticity,regulate autophagy apoptosis and related signal pathways,regulate peripheral oxidation and other aspects,in order to explore the mechanism of acupuncture treatment of CSR,in order to provide some reference for clinical treatment of CSR.
