摘 要:目的分析射血分数保留心力衰竭(HFpEF)患者超声左心房容积指数(LAVI)与心脏周围脂肪厚度的关系,探讨心脏周围脂肪厚度对HFpEF的潜在预测价值。方法选取HFpEF患者72例作为HFpEF组,另选取无心力衰竭病史的健康体检者69例作为对照组。使用彩色超声诊断仪对受试者行超声心动图检查,采用改良Simpson法检测受试者LAVI,于舒张末期测量受试者右心室前壁心外膜脂肪(EAT)厚度、右心室前壁心包脂肪(PAT)厚度及左心房室沟EAT厚度。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),分析HFpEF患者右心室前壁EAT厚度、右心室前壁PAT厚度及左心房室沟EAT厚度对LAVI≥34 mL/m^(2)的预测效能;采用Spearman相关分析HFpEF患者右心室前壁EAT厚度、右心室前壁PAT厚度及左心房室沟EAT厚度与LAVI的相关性。结果HFpEF组LAVI、右心室前壁EAT厚度、右心室前壁PAT厚度和左心房室沟EAT厚度均高于对照组(P均<0.05)。ROC曲线分析结果显示,右心室前壁EAT厚度、右心室前壁PAT厚度及左心房室沟EAT厚度预测LAVI≥34 mL/m^(2)的AUC分别为0.710、0.508、0.684。Spearman相关性分析结果显示,右心室前壁EAT厚度、左心房室沟EAT厚度与LAVI呈正相关(r分别为0.334、0.275,P均<0.01),右心室前壁PAT厚度与LAVI之间无相关性(r=0.060,P=0.477)。结论HFpEF患者超声LAVI与右心室前壁EAT厚度、左心房室沟EAT厚度呈正相关,其中右心室前壁EAT厚度与LAVI相关性更高,或可成为评估HFpEF的相关指标之一。Objective To analyze the relationship between ultrasound left atrial volume index(LAVI)and pericardial fat thickness in patients with heart failure with preserved ejection fraction(HFpEF),and to explore the potential predictive value of pericardial fat thickness for HFpEF.Methods Seventy-two patients with HFpEF were selected as the HFpEF group,and 69 healthy individuals without a history of heart failure were selected as the control group.Echocardiography was performed on the subjects using a colour ultrasound diagnostic instrument,and LAVI was detected using the modified Simpson method.The measurements of epicardial adipose tissue(EAT)thickness on the anterior wall right ventricular,paracardial adipose tissue(PAT)thickness on the anterior wall of right ventricular,and EAT thickness in the left atrioventricular groove were obtained at end-diastole.The receiver operating characteristic(ROC)curves were plotted,and the area under the curve(AUC)was calculated to analyse the predictive efficacy of right ventricular anterior wall EAT thickness,right ventricular anterior wall PAT thickness and left atrioventricular groove EAT thickness on LAVI≥34 mL/m^(2)in patients with HFpEF;Spearman correlation analysis was employed to examine correlations between the right ventricular anterior wall EAT thickness,right ventricular anterior wall PAT thickness,left atrioventricular EAT thickness and LAVI in HFpEF patients.Results LAVI,right ventricular anterior wall EAT thickness,right ventricular anterior wall PAT thickness and left atrioventricular groove EAT thickness were higher in the HFpEF group than in the control group(all P<0.05).ROC curve analysis showed that the AUC of the right ventricular anterior wall EAT thickness,right ventricular anterior wall PAT thickness and left atrioventricular groove EAT thickness in the prediction of LAVI≥34 mL/m^(2)was 0.710,0.508,and 0.684.Spearman correlation analysis showed that the right ventricular anterior wall EAT thickness,left atrioventricular groove EAT thickness were positively c
