摘　　要：目的 探讨改良经脐单孔腹腔镜技术对异位妊娠的临床效果。方法 选取2021年9月—2023年2月在本院妇科住院治疗的异位妊娠手术患者120例,随机分为A组和B组,每组60例。其中,A组患者采用改良经脐单孔腹腔镜手术治疗,B组患者采用传统腹腔镜手术治疗,比较两组手术时间、术中出血量、有无输血、术后肛门排气情况、术后伤口疼痛评分、术后止痛剂使用情况、有无发热、住院天数等指标。结果 A组手术时间明显长于B组(P<0.05)。两组术中出血量、术后肛门排气时间、住院时间差异无统计学意义(P>0.05)。两组术中均无输血、术后均无发热,A组术后24 h和术后72 h的疼痛评分均显著低于B组(P<0.05)。A组术后止痛剂使用率明显低于B组(P<0.05)。结论 在异位妊娠手术治疗中应用改良经脐单孔腹腔镜技术可减轻术后疼痛,减少术后止痛剂使用率,并不影响术中出血量和术后恢复进程,且腹部美观,具有临床推广价值。Objective To explore clinical effect of improved transumbilical single port laparoscopy for ectopic pregnancy.Methods The paper chose 120 patients with ectopic pregnancy with surgical treatment in our gynecology department from September 2021 to February 2023,and divided them into group A and B randomly,with 60 cases in each group.Group A was treated with improved transumbilical single port laparoscopic surgery,while group B with traditional laparoscopic surgery.Surgical time,intraoperative blood loss,blood transfusion,postoperative anal exhaust,postoperative wound pain scores,postoperative analgesic use,fever,length of stay and other indicators were compared between two groups.Results Surgery time in group A was significantly longer than group B(P<0.05),there was no statistically significant difference in intraoperative blood loss,postoperative anal exhaust time and length of stay between two groups(P>0.05).There was no blood transfusion during surgery and no fever after surgery in two groups.Pain scores 24 hours and 72 hours after surgery in group A were significantly lower than group B(P<0.05),use of analgesics in group A was significantly lower than group B(P<0.05).Conclusion Improved transumbilical single port laparoscopic technique can alleviate postoperative pains of ectopic pregnancy with surgical treatment,reduce use of analgesics,does not affect intraoperative bleeding or postoperative recovery process,with good-looking abdomen,which has clinical value for promotion and application.
