自拟痛经贴联合热敏灸治疗原发性痛经(寒凝血瘀型)即时止痛作用的临床观察

摘　　要：目的:观察自拟痛经贴联合热敏灸治疗寒凝血瘀型原发性痛经患者的即时止痛作用,期望为今后的临床治疗提供新思路。方法:将90例患者随机分为自拟痛经贴联合热敏灸组(治疗1组)、热敏灸组(治疗2组)和西药组(对照组),各30例,于第1个月经周期痛经第1天观察3组的即时止痛时间,连续治疗3个月经周期,并于疗程结束后第1个月随访患者腹痛程度、中医证候评分及总有效率,治疗完成后第3及6个月随访复发率,对比3组的即时止痛及远期疗效。结果:在即时止痛方面,治疗1组与治疗2组比较,差异无统计学意义(P>0.05),但2组的即时止痛时间都短于对照组(P<0.05);在总有效率及治疗后3个月复发率方面,治疗1组与治疗2组比较,差异无统计学意义(P>0.05),但2组均优于对照组(P<0.05);在降低腹痛程度、中医证候评分及治疗后6个月复发率方面,治疗1组优于其他2组(P<0.05),且治疗2组优于对照组(P<0.05)。结论:对于寒凝血瘀型原发性痛经患者,自拟痛经贴联合热敏灸疗法的即时止痛及远期临床疗效显著,可明显缩短患者疼痛时间、减轻腹痛程度及中医症状、增加总有效率及降低复发率。Objective:To observe the immediate analgesic effect of self-made dysmenorrhea paste combined with heat-sensitive moxibustion in the treatment of primary dysmenorrhea patients with cold coagulation and blood stasis,and to provide new ideas for future clinical treatment.Methods:90 patients were randomly divided into self-made dysmenorrhea paste combined with heat-sensitive moxibustion group(treatment group 1),heat-sensitive moxibustion group(treatment group 2)and western medicine group(control group),30 cases in each group.The immediate analgesic relief time of the three groups was observed on the first day of dysmenorrhea in the first menstrual cycle,and the three menstrual cycles were treated continuously.The degree of abdominal pain,TCM syndrome score and total effective rate were followed up one month after the end of the course of treatment.The recurrence rate was followed up at 3 and 6 months after the completion of treatment,and the immediate pain relief and long-term efficacy of the three groups were compared.Results:In terms of immediate analgesia,there was no significant difference between the treatment group 1 and the treatment group 2(P>0.05),and the immediate analgesia time of the two groups was shorter than that of the control group(P<0.05).The total efficiency and recurrence rate at 3 months after treatment in the treatment group 1 and the treatment group 2 were better than those in the control group(P<0.05),while there was no significant difference in the total efficiency and recurrence rate at 3 months after treatment between the treatment group 1 and the treatment group 2(P>0.05).In terms of reducing the degree of abdominal pain,TCM syndrome score and recurrence rate at 6 months after treatment,the treatment group 1 was better than the other two groups(P<0.05),and the treatment group 2 was better than the control group(P<0.05).Conclusion:For patients with primary dysmenorrhea of cold coagulation and blood stasis type,the immediate analgesic and long-term clinical efficacy of self-made dysmenorrhea
