摘　　要：目的探讨急性缺血性脑卒中患者机械取栓后住院期间发生下肢深静脉血栓(DVT)的危险因素。方法收集2020年1月至2023年1月于首都医科大学附属北京友谊医院行机械取栓的129例急性缺血性脑卒中患者的临床资料,按照下肢DVT的发生情况将其分为DVT组(n=31)和对照组(n=129)。收集所有患者入院时的一般资料、机械取栓时的介入手术相关指标、术后住院期间的复查结果以及下肢DVT的发生情况,分析急性缺血性脑卒中患者机械取栓后住院期间发生下肢DVT的影响因素。结果老年(年龄﹥60岁)、发病至取栓时间﹥6 h、入院时合并下肢偏瘫、入院时合并意识障碍均是急性缺血性脑卒中患者机械取栓后住院期间发生下肢DVT的独立危险因素(P﹤0.05)。结论老年(年龄﹥60岁)、发病至取栓时间﹥6 h、入院时合并下肢偏瘫、入院时合并意识障碍均是急性缺血性脑卒中患者机械取栓后住院期间发生下肢DVT的危险因素,临床中的高危患者需加强机械性预防。Objective To investigate the risk factors of lower extremity deep vein thrombosis(DVT)in patients with acute ischemic stroke during hospitalization after mechanical thrombectomy.Method Clinical data of 129 patients with acute ischemic stroke who underwent mechanical thrombectomy in Beijing Friendship Hospital,Capital Medical University from January 2020 to January 2023 were collected.According to the occurrence of DVT in lower extremity,they were divided into DVT group(n=31)and control group(n=129).The general data of all patients at admission,relevant indexes of interventional surgery during mechanical thrombectomy,reexamination results during postoperative hospitalization and the incidence of DVT in lower limbs were collected,and the influencing factors of DVT in lower extremity during hospitalization after mechanical thrombectomy in patients with acute ischemic stroke were analyzed.Result Old age(age>60 years old),from onset to thrombolysis time>6 h,lower extremity hemiplegia at admission,and consciousness disturbance at admission were all independent risk factors for lower extremity DVT in patients with acute ischemic stroke after mechanical thrombolysis(P<0.05).Conclusion Old age(age>60 years old),from onset to thrombectomy time>6 h,lower extremity hemiplegia at admission,and consciousness disturbance at admission are all risk factors for lower limb DVT in patients with acute ischemic stroke after mechanical thrombectomy.Mechanical prevention should be strengthened in high-risk patients in clinic.
