摘　　要：目的:调查初产妇产后泌乳启动延迟(DOL)发生情况并分析相关影响因素。方法:选择2022年1月-2024年3月在本院分娩的初产妇为研究对象,根据产妇分娩后72h内主诉未有乳房胀满感和充盈感判定DOL,收集相关临床资料,利用logistic回归分析初产妇产后DOL发生的独立危险因素。结果:纳入初产妇3038例,其中1014例(33.4%)产后发生DOL。logistic回归分析显示,年龄≥35岁(OR=2.308,P=0.008)、患GDM(OR=3.473,P=0.006)、患HDP(OR=4.489,P=0.003)、患SCH(OR=2.798,P=0.000)、胎膜早破(OR=1.773,P=0.014)、剖宫产(OR=1.317,P=0.011)是初产妇产后DOL的独立危险因素(均P<0.05)。结论:本次调查的初产妇产后DOL发生率较高,发生危险因素包括年龄≥35岁、GDM、HDP、SCH、胎膜早破、剖宫产,医护人员应重视有上述特点的初产妇并及时干预,预防产后DOL发生。Objective:To investigate the occurrence and the related influencing factors of the delayed onset of lactogenesis(DOL)of primiparas in the delivery room.Methods:The primiparas who gave birth in the hospital from January 2022 to March 2024 were selected as the research subjects.The DOL of these primiparas was determined according to the primiparas without the feelings of the breast tumescence and fullness within 72 hours after delivery.The relevant clinical data of the primiparas with DOL were collected.Logistic regression was used to analyze the independent risk factors of the postpartum DOL occurrence of the primiparas.Results:A total of 3038 primiparas in the delivery room were enrolled in this study,and there were 1014(33.4%)cases with the postpartum DOL.Logistic regression analysis showed that the age≥35 years old(OR=2.308,P=0.008),the GDM(OR=3.473,P=0.006),the HDP(OR=4.489,P=0.003),the SCH(OR=2.798,P=0.000),the premature rupture of membranes(OR=1.773,P=0.014)and the cesarean section(OR-1.317,P=0.011)of the primiparas were the independent risk factors of their postpartum DOL occurrence in the delivery room(all P<o.05).Conclusion:The incidence of the postpartum DOL of the primiparas in the delivery room is higher.The independent risk factors of the postpartum DOL of the primiparas include the age≥35 years old,GDM,HDP,SCH,premature rupture of membranes and cesarean section.The medical staff should pay attention to the primiparas with these independent risk factors,and the effective intervention measures should be conducted for these primiparas timely to prevent their postpartum DOL occurrence.
