基于故事理论的叙事护理对肝癌术后患者心理弹性及创伤后成长水平的影响

摘　　要：目的:探讨基于故事理论的叙事护理对肝癌术后患者心理弹性及创伤后成长水平的影响。方法:选取2022年1月至2024年1月我院收治的肝癌术后患者152例,采用随机数字表法将患者分为对照组(n=76)与研究组(n=76)。对照组予以常规干预,研究组在常规干预的基础上予以基于故事理论的叙事护理,均干预2个月。比较两组心理弹性(CD-RISC)、创伤后成长水平(PTGI)及应对方式(MCMQ)等评分。结果:研究组干预后CD-RISC各维度得分较对照组高(t=8.568、4.867、8.584,均P<0.001);研究组干预后PTGI各得分较对照组高(t=8.314、8.935、13.322、10.740、7.849,均P<0.001);研究组干预后面对得分较对照组高(t=8.345,P<0.001),研究组干预后回避、屈服得分较对照组低(t=7.716、6.541,均P<0.001)。结论:基于故事理论的叙事护理可改善肝癌术后患者心理弹性,促进患者积极面对疾病,提高其创伤后成长水平。Objective:To explore the influence of narrative nursing based on story theory on the psychological resilience and post-traumatic growth level in patients with liver cancer after operation.Methods:A total of 152 postoperative patients with liver cancer treated in our hospital from January 2022 to January 2024 were selected and divided into control group(n=76)and study group(n=76)by random number table method.The control group was given routine intervention,while the study group was given narrative nursing based on story theory besides routine intervention.Both groups were intervened for 2 months,and the scores of psychological resilience(connor-Davidson resilience scale,CD-RISC),post-traumatic growth level(post-traumatic growth inventory,PTGI)and coping style(medical coping modes questionnaire,MCMQ)were compared between the two groups.Results:After intervention,the scores of CD-RISC in the study group were higher than those in the control group(t=8.568,4.867,8.584,all P<0.001).The scores of PTGI in the study group were higher than those in the control group after intervention(t=8.314,8.935、13.322,10.740,7.849,all P<0.001).After the intervention,the score of confrontation in the study group was higher than that in the control group(t=8.345,P<0.001),while the scores of avoidance and yield of the study group were lower than those of the control group(t=7.716,6.541,both P<0.001).Conclusion:Narrative nursing based on story theory can improve the psychological resilience of patients with liver cancer after operation,promote patients to face the disease actively and improve their post-traumatic growth level.
