改进式手术室全期护理在全子宫切除手术中的应用研究

摘　　要：目的:探究改进式手术室全期护理在全子宫切除手术患者中的应用效果。方法:将2021年7月至2023年6月于东台市中医院行全子宫切除术的84例患者按照入院顺序采用奇偶数分组法分为常规组与改进组,各42例。常规组予以常规手术室护理,改进组在常规组基础上予以改进式手术室全期护理。比较术中及术后并发症情况、术后恢复情况、护理质量及手术前后凝血功能[D-二聚体(D-D)、纤维蛋白原(Fib)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)]。结果:改进组APTT、PT、D-D、Fib术后与术前之间差值均低于常规组(P<0.05);改进组与常规组术中并发症发生率、术后并发症发生率比较,差异无统计学意义(P>0.05);改进组引流管拔除时间、胃肠道恢复时间及住院时间均较常规组短,术后24 h疼痛评分较常规组低(P<0.05);改进组护理质量各项评分均较常规组高(P<0.05)。结论:对全子宫切除术患者实施改进式手术室全期护理可改善凝血功能,促进术后恢复,提高护理质量。Objective:To explore the application effectiveness of improved operating room full-term nursing in patients undergoing total hysterectomy.Methods:A total of 84 patients who underwent total hysterectomy in the Dongtai Hospital of Traditional Chinese Medicine from July 2021 to June 2023 were divided into routine group and improved group according to their admission order using odd-even number grouping method,with 42 cases in each group.The routine group was given routine operating room nursing,and the improved group was given improved operating room full-term nursing on the basis of the routine group.The intraoperative and postoperative complications,postoperative recovery,nursing quality and coagulation function[D-dimer(D-D),fibrinogen(Fib),activated partial thromboplastin time(APTT),prothrombin time(PT)]before and after operation were compared.Results:The difference between APTT,PT,D-D and Fib levels before and those after operation in the improved group was lower than that in the conventional group(P<0.05).There was no significant difference in the incidence of intraoperative complications and postoperative complications between the improved group and the conventional group(P>0.05).The time of drainage tube removal,gastrointestinal recovery and hospitalization in the improved group was shorter than that in the routine group,and the pain score at 24 h after operation was lower than that in the routine group(P<0.05).The scores of nursing quality in the improved group were higher than those in the routine group(P<0.05).Conclusion:Improved operating room full-term nursing for patients undergoing total hysterectomy can improve coagulation function,promote postoperative recovery and improve the nursing quality.
