摘 要:目的明确北京地区儿童感染洋葱伯克霍尔德菌复合体(BCC)的流行趋势及BCC菌株的基因组和表型特征。方法收集269份肺炎儿童的临床样本,同时采用分离培养和等温扩增快速筛查方法评估BCC的流行特征;通过系统进化分析、毒力基因筛查、毒力表型实验和耐药表型实验明确BCC菌株的基因组、毒力和耐药特征。结果BCC的分离阳性率为1.12%,快速筛查方法的阳性率为2.60%。在3株分离株中,2株属于洋葱伯克霍尔德菌,携带更多的毒力基因,但细胞毒性较低,且耐药较为严重;1株属于越南伯克霍尔德菌,携带更少的毒力基因,但细胞毒性较高,且仅对1种抗生素耐药。2个菌种的毒力表型差异是否与基因katG有关,有待后续研究进一步探究。结论本研究对北京地区近3年的儿童BCC感染情况进行了调查,并明确了BCC菌株的基因组、系统进化和表型特征,为BCC的预防控制提供了详实的基础数据支撑。Objective To clarify the prevalence of burkholderia cepacia complex(BCC)among children in Beijing and to investigate the genomic and phenotypic characteristics of BCC isolates.Methods A total of 269 clinical samples from children with pneumonia were collected.The prevalence of BCC was evaluated using both isolation culture and isothermal amplification rapid screening methods.Genomic,virulence,and antibiotic-resistance features of BCC isolates were identified through phylogenetic analysis,virulence gene screening,virulence phenotype experiment,and antibiotic-resistance phenotype experiment.Results The positive rate of BCC isolation was 1.12%,while the positive rate of the rapid screening method was 2.60%.Among the three isolated isolates,two belonged to burkholderia cepacia,carrying more virulence genes but showing lower cytotoxicity and more severe antibiotic-resistance;and one belonged to burkholderia vietnamiensis,carrying fewer virulence genes but exhibiting higher cytotoxicity and resistance to only one antibiotic.Whether the difference in virulence phenotypes between the two species is related to the gene katG requires further verification.Conclusion This study investigated the infection situation of BCC among children in Beijing over the past three years and elucidated the genomic,phylogenetic,and phenotypic characteristics of BCC isolates,providing detailed foundational data support for the prevention and control of BCC infection.
