慢性自发性荨麻疹患者外周血25-羟基维生素D、IL-10、IL-17检测价值及其关联性研究

摘　　要：目的分析慢性自发性荨麻疹(CSU)患者血清25-羟基维生素D[25(OH)D]、白介素-10(IL-10)、白介素-17(IL-17)的表达水平,探讨维生素D、IL-10及IL-17与慢性自发性荨麻疹的关系及其临床检测价值。方法选取慢性自发性荨麻疹患者47例作为病例组,对照组为同期体检的37例健康志愿者。比较病例组与对照组血清25(OH)D、IL-10、IL-17水平,分析CSU患者活动性评分与血清25(OH)D、IL-10、IL-17的相关性。结果CSU患者及健康对照组外周血中的25(OH)D水平均低于正常值,CSU组外周血中25(OH)D高于对照组(P<0.01)。病例组外周血IL-17表达水平明显高于对照组,IL-10表达水平相对对照组降低(P<0.01)。病例组血清25(OH)D与IL-10、IL-17水平之间均无相关性,而IL-10与IL-17水平呈负相关(r=-0.377,P<0.05)。病例组患者血清25(OH)D(r=-0.313,P<0.05)、IL-10(r=-0.459,P<0.01)水平与活动性评分呈负相关,而IL-17水平与活动性评分呈正相关(r=0.749,P<0.01)。结论CSU患者外周血IL-10、IL-17水平与病情进展相关,IL-10、IL-17可作为判断CSU发生发展的辅助指标;25(OH)D可能参与了CSU发生发展,但在本地区CSU患者中外周血25(OH)D的临床检测意义可能相对较小。Objective To evaluate the serum concentrations of 25-Hydroxyvitamin D[25(OH)D],Interleukin-10(IL-10),and Interleukin-17(IL-17)in the peripheral blood of patients diagnosed with chronic spontaneous urticaria(CSU).Additionally,the interrelationship among these biomarkers was elucidated to evaluate their potential clinical significance in diagnosis and management.Methods The case group consisted of 47 patients with CSU.Meanwhile,the control group included 37 healthy volunteers who underwent physical examinations during the same period.The levels of serum 25(OH)D,IL-10,and IL-17 in peripheral blood were compared between the case group and control group.The correlation between serum 25(OH)D,IL-10,IL-17 levels in peripheral blood and symptom scores of CSU patients were analyzed.Results The serum 25(OH)D levels in both the CSU patient group and the healthy control group were below the normal range.However,the peripheral blood 25(OH)D level in the CSU group was significantly higher than that in the control group(P<0.01).Additionally,the expression levels of IL-17 in peripheral blood were significantly elevated in the CSU group compared to the control group,while the expression levels of IL-10 were significantly lower(P<0.01).There was no correlation between serum 25(OH)D and IL-10 and IL-17 levels in case group,but there was a negative correlation between serum IL-10 and IL-17 levels in case group(r=-0.377,P<0.05).The serum 25(OH)D level was negatively correlated with symptom score in case group(r=-0.313,P<0.05).The serum IL-10 level was negatively correlated with symptom score in case group(r=-0.459,P<0.01).Serum IL-17 level was positively correlated with symptom score in patients with CSU(r=0.647,P<0.01).Conclusion The levels of IL-10 and IL-17 in peripheral blood of CSU patients are correlated with the progression of the disease,and IL-10 and IL-17 can be used as auxiliary indicators to judge the occurrence and development of CSU.25(OH)D may be involved in the development of CSU,but the clinical significance of 25(OH)
