摘 要:目的旨在介绍一种新开发的牵开系统,并分析其在显微镜辅助经前路颈椎间盘切除与融合术(ACDF)治疗颈椎病的可行性和安全性。方法回顾性收集并分析2019年1月至2023年11月于遵义医科大学附属医院诊断为颈椎病,且使用新开发的牵开系统进行显微镜辅助下ACDF的患者资料,评估该牵开系统的可行性及安全性。结果本研究共纳入48例患者。术后影像学数据显示,减压充分,手术效果良好。视觉模拟评分(VAS)和颈部残疾指数评分(NDI)分别从术前的(5.43±1.12、29.77±6.06)分下降至术后的(2.60±0.78、11.75±3.26)分。日本骨科协会评分(JOA)从术前的(7.93±1.37)分上升至术后的(12.22±1.71)分。C2-7的Cobb角从术前的(10.37±6.79)度增加到手术后(14.58±6.10)度。未观察到临床或影像学相关并发症。结论新开发的牵开系统在显微镜辅助下ACDF手术中表现出良好的可行性和安全性,可为临床应用提供一种新的选择。Objective This study seeks to introduce a newly developed retraction system and analyze its feasibility and safety in the context of microscope-assisted anterior cervical discectomy and fusion(ACDF).Methods Patients diagnosed with cervical spondylosis in the Affiliated Hospital of Zunyi Medical University from Jan 2019 to Nov 2023 were included and the newly developed retraction system was used in microscope-assisted ACDF for treatment.The relevant demographic data,as well as preoperative,intraoperative,and postoperative clinical and imaging data were collected and retrospectively analyzed to assess outcomes and potential risks.Results A total of 48 patients were included in this study.Postoperative imaging demonstrated sufficient decompression and confirmed favorable surgical outcomes.The reoperative visual analog scale(VAS)score and neck disability index(NDI)score decreased from(5.43±1.12,29.77±6.06)to(2.60±0.78,11.75±3.26),respectively.The Japanese Orthopaedic Association(JOA)score increased from(7.93±1.37)preoperatively to(12.22±1.71)postoperatively,indicating improved neurological function.The Cobb angle from C2 to C7 increased from(10.37±6.79)degrees to(14.58±6.10)degrees postoperatively.No clinically or radiologically relevant complications were observed during the study period.Conclusion The newly developed retraction system demonstrated both good feasibility and safety in microscope-assisted ACDF,which provides a viable option for clinical use.
