摘 要:疼痛严重影响着患者的生活质量与身心健康,并可能引发或加剧焦虑、抑郁等情绪问题,但疼痛所涉及的复杂神经环路及其作用机制仍是一个亟待解决的科学问题。痛信息在中枢内的传递主要依赖于丘脑外侧系统和内侧系统。其中,来自脊髓或脑干的痛信息在经臂旁核(PBN)中继后,能够通过丘脑内侧系统的背侧中线和内侧核团(TMMN)进一步上传到大脑边缘皮质和皮质下结构。TMMN包括丘脑室旁核(PVT)、丘脑中央内侧核(CM)、丘脑背内侧核(MD)、丘脑连接核(Re)和丘脑菱形核(Rh),通过与前额叶皮质(PFC)、岛叶(IC)、杏仁核簇(AMY)等核团的联系参与痛信息及相关负性情绪的传递与调控。本综述总结了TMMN各核团在参与痛觉感受和情绪调节过程中神经元活性的变化以及相关神经通路和机制,以期为疼痛调控神经机制后续研究提供参考。Pain seriously affects the quality of life and physical and mental health of patients,and may cause or aggravate anxiety,depression and other emotional problems,but the complex neural circuits involved in pain and its mechanisms are unsolved scientific problems.The transmission of pain information in the central nervous system depends on the thalamic lateral system and the thalamic medial system.In the thalamic medial system,after pain information from the spinal cord or brain stem is relayed in the parabrachial nucleus(PBN),it can be further terminated in the limbic cortex and subcortical structures of the cerebrum through the dorsal thalamic midline and medial nuclei(TMMN).TMMN includes paraventricular thalamic nucleus(PVT),central medial thalamus nucleus(CM),mediodorsal thalamic nucleus(MD),reuniens thalamic nucleus(Re)and rhomboid thalamic nucleus(Rh).TMMN is involved in the transmission and regulation of pain information and related negative emotions through its connection with prefrontal cortex(PFC),insular cortex(IC),amygdaloid complex(AMY)and other nuclei.This review summarizes the changes of neuronal activity and related neural pathways and mechanisms in TMMN involved in pain perception and emotion regulation,in order to provide reference for subsequent research on the neural mechanisms of pain regulation.
