摘　　要：目的 分析特定电磁波谱(TDP)照射联合多磺酸粘多糖乳膏外涂对血液透析患者人造血管动静脉内瘘使用时间、生活质量的影响。方法 选取2017年1月至2018年12月我院收治的84例血液透析患者作为研究对象,利用随机双盲法将其分为对照组和研究组,每组42例。所有患者均接受人造血管动静脉内瘘植入术。对照组给予多磺酸粘多糖乳膏外涂,研究组在对照组基础上联合TDP照射。比较两组的干预效果。结果干预3个月后及随访1、2、3年,研究组的内瘘血流量大于对照组(P<0.05)。研究组的血管弹性、血管杂音、血管流量、血管塌陷均明显优于对照组(P<0.05)。随访末期,研究组的精神健康、情感职能、活力、总体健康、生理功能评分均高于对照组(P<0.05)。研究组的动静脉内瘘使用时间≥3年占比高于对照组,平均使用时间长于对照组(P<0.05)。结论 TDP照射联合多磺酸粘多糖乳膏外涂应用于血液透析患者中的效果显著,可有效提高人造血管动静脉内瘘质量,维持良好的血流量,并延长内瘘使用时间,改善患者生存质量,值得推广。Objective To analyze the effects of thermal design power(TDP)irradiation combined with external application of mucopolysaccharide polysulfate cream on the use time of arteriovenous graft and quality of life in hemodialysis patients.Methods A total of 84 hemodialysis patients admitted in our hospital from January 2017 to December 2018 were selected as the research objects.The patients were divided into control group and study group by random double-blind method,with 42 cases in each group.All patients underwent arteriovenous graft implantation.The control group was given external application of mucopolysaccharide polysulfate cream,and the study group was combined with TDP irradiation on the basis of the control group.The intervention effects of the two groups were compared.Results After 3 months of intervention and 1,2 and 3 years of follow-up,the blood flow of internal fistula in the study group was greater than that in the control group(P<0.05).The vascular elasticity,vascular murmur,vascular flow and vascular collapse of the study group were significantly better than those of the control group(P<0.05).At the end of follow-up,the scores of mental health,emotional function,vitality,general health and physiological function in the study group were higher than those in the control group(P<0.05).The proportion of arteriovenous fistula use time≥3 years in the study group was higher than that in the control group,and the average use time was longer than that in the control group(P<0.05).Conclusion The TDP irradiation combined with external application of mucopolysaccharide polysulfate cream on hemodialysis patients has significant effect,it can effectively elevate the quality of arteriovenous graft,maintain good blood flow,prolong the use time of internal fistula,and improve the quality of life of patients,which is worth promoting.
