甘草泻心汤联合滋阴降火解毒汤治疗乳腺癌术后化疗所致口腔黏膜炎60例

摘　　要：目的:观察甘草泻心汤联合滋阴降火解毒汤治疗乳腺癌术后化疗所致口腔黏膜炎的疗效及对炎性因子的影响。方法:选取2019年10月到2022年10月洛阳市中医院收治的乳腺癌术后化疗所致口腔黏膜炎患者120例,随机分成观察组和对照组,每组60例。两组患者均给予基础口腔护理,并给予康复新液含漱,对照组另给予滋阴降火解毒汤治疗,观察组在对照组治疗的基础上加用甘草泻心汤治疗。观察两组治疗前后口腔黏膜炎毒性分级、中医证候积分、血清炎症因子[肿瘤坏死因子(tumor necrosis factor, TNF)-α、白细胞介素(interleukin, IL)-2及IL-8]水平及临床疗效。结果:观察组有效率为94.83%,对照组有效率为82.14%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后口腔黏膜炎毒性分级情况优于对照组,差异有统计学意义(P<0.05);观察组治疗后口干口渴、口疮、手足心热、失眠多梦及大便干结等中医证候积分低于对照组,差异有统计学意义(P<0.05);观察组治疗后TNF-α、IL-8水平低于对照组,IL-2水平高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为5.17%,对照组不良反应发生率为3.57%,两组不良反应发生率比较,差异无统计学意义(P>0.05)结论:甘草泻心汤联合滋阴降火解毒汤治疗乳腺癌术后化疗所致口腔黏膜炎,能显著改善患者口腔黏膜炎毒性分级、中医证候,降低炎症反应。Objective:To observe the clinical efficacy of Licorice Heart-Draining Decoction combined with Yin-Nourishing and Fire-Reducing and Detoxication Decoction on oral mucositis after chemotherapy of breast cancer and its effect on inflammatory factors.Methods:A total of 120 patients with oral mucositis after chemotherapy of breast cancer in Luoyang Hospital of Traditional Chinese Medicine from October 2019 to October 2022 were randomly divided into the observation group and the control group,with 60 cases in each group.The two groups were first given basic oral care and Kangfuxin Liquid for gargling,then the control group was additionally given Yin-Nourishing and Fire-Reducing and Detoxication Decoction,while the observation group was treated further with Licorice Heart-Draining Decoction on the basis of the treatment of the control group.The toxicity grading of oral mucositis,TCM syndrome scores,serum inflammatory factors[including tumor necrosis factor(TNF)-α,interleukin(IL)-2 and IL-8]and clinical efficacy of the two groups were observed before and after treatment.Results:The effective rate of the observation group was 94.83%,while that of the control group was 82.14%.The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the toxicity grading of oral mucositis of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05);the scores of TCM syndromes such as dry mouth and thirst,mouth sore,hot heart in hands and feet,insomnia and dreaminess,and dry stool in the observation group were lower than those in the control group,and all the differences were statistically significant(P<0.05);the levels of TNF-αand IL-8 in the observation group were lower than those in the control group,while the level of IL-2 was higher than that in the control group,and the difference was statistically significant(P<0.05);the incidence of adverse reactions in the observation g
