摘 要:目的:探讨自动预置(Auto-prescription)技术结合不同权重自适应统计迭代重建算法(ASIR-V)在腹壁下动脉CTA中的应用价值。方法:前瞻性搜集于我院Revolution CT行腹壁下动脉CTA检查的患者80例,身体质量指数(BMI)≤29.00 kg/m^(2),随机分为两组。A组(40例)采用常规120 kVp结合40%ASIR-V重建;B组(40例)采用Auto-prescription技术结合40%、60%、80%的ASIR-V重建,获得B1~B3三个亚组的图像。在各组轴面图像上测量髂外动脉及同层面腰大肌的CT值和SD值,计算信噪比(SNR)及对比噪声比(CNR),统计A、B两组的辐射剂量。由2位观察者采用盲法独立在各组VR和MIP图上对腹壁下深动脉及腹壁下浅动脉行3分制评分,图像噪声和伪影行5分制评分。结果:B组髂外动脉CT值与A组相比提升了24%(t=-4.094~-4.053,P<0.001)。B组内随着ASIR-V权重的提高,图像SNR、CNR逐渐升高(F=21.610、30.863,P<0.001)。B2和B3组的SNR、CNR均优于A组(t=-5.521~4.497,P<0.05)。2位观察者主观评价的一致性较好(Kappa值0.774~0.947,P<0.05),图像噪声和伪影主观评分A组与B2、B3组间差异无统计学意义。B1~B3组图像在腹壁下深动脉穿支、穿出点显示、肌内走行和腹壁下浅动脉显示等方面的主观评分均优于A组,其中B2组得分最高。B组辐射剂量与A组相比平均降低了23.8%。结论:采用Auto-prescription技术结合60%ASIR-V算法能够显著优化腹壁下动脉的显示并在一定程度上降低辐射剂量。Objective:The purpose of this study was to explore the application value of the auto-prescription technique combined with different weights adaptive statistical iterative reconstruction algorithm(ASIR-V)in CTA of the deep inferior epigastric artery.Methods:A total of 80 patients with body mass index(BMI)≤29.00kg/m 2 who underwent deep inferior epigastric arterial CT angiography at the revolution CT scanner in our hospital were prospectively collected.All subjects were randomly assigned into two groups.Group A(n=40)was submitted to conventional 120kVp combined with 40%ASIR-V reconstruction.Group B(n=40)obtained three subgroups B1~B3 by using auto-prescription technology combined with 40%,60%and 80%ASIR-V reconstruction.The CT and SD values of the external iliac artery and psoas major muscle at the same level in each group were measured.The signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)were calculated.The radiation doses of Group A and Group B were recorded.The deep inferior epigastric artery and superficial inferior epigastric artery were scored on a 3-point scale and the image noise and artifacts were scored on a 5-point scale by two observers independently on the VR and MIP images of each group.Results:The CT values of the external iliac artery in group B were 24%higher than those in group A(t=-4.094~-4.053,P<0.001).In group B,SNR and CNR increased gradually with the increase of ASIR-V level(F=21.610,30.863,P<0.001).The SNR and CNR in groups B2 and B3 were better than those in group A(t=-5.521 to 4.497,P<0.05).The two observers had good consistency in the evaluation of image quality(Kappa value:0.774~0.947,P<0.05).There was no significant difference in the subjective score of image noise and artifact between groups A and B2 and B3.In the images of groups B1 to B3,the subjective scores of perforating vessels from the deep inferior epigastric artery,the point of emergence from the fascia of the rectus abdominis muscle,intramuscular course,and superficial inferior epigastric artery display were all be
