MIS-TLIF与传统开放术式在单节段腰椎退行性患者治疗中的临床对比研究

摘　　要：目的:对比微创经椎间孔入路腰椎椎体间融合术(MIS-TLIF)与传统开放术式在单节段腰椎退行性患者治疗中的效果。方法:选取2021年1月—2023年11月青海省第五人民医院收治的68例单节段腰椎退行性患者作为研究对象,根据不同的手术方式分组,将行MIS-TLIF术的36例患者纳入A组,将行传统开放经椎间孔椎间融合术(TLIF)的32例患者纳入B组。观察两组手术情况,康复进程,术后腰椎功能、疼痛程度及术后并发症。结果:A组手术时间、术中出血量、术后出血量、术后首次下地时间和住院总时间明显少于B组,差异有统计学意义(P<0.05);两组术后1个月、3个月时的视觉模拟量表(VAS)、Oswestry功能障碍指数(ODI)、改良日本骨科协会(M-JOA)评分明显低于术前,且A组评分明显低于B组,差异有统计学意义(P<0.05);术后3个月内A组并发症总发生率明显低于B组,差异有统计学意义(P<0.05)。结论:单节段腰椎退行性患者经MIS-TLIF术治疗相较行传统开放术治疗可有效缩短手术时间,减少出血量,加快康复进程,减轻疼痛感,促进腰椎功能的恢复,且术后并发症少。Objective:To compare the effect of minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF)and traditional open surgery in the treatment of patients with single-segment lumbar degenerative disease.Method:68 patients with single-segment lumbar degenerative disease admitted tp the Fifth People's Hospital of Qinghai Province from January 2021 to November 2023 were selected as the study objects.According to different surgical methods,36 patients who received MIS-TLIF were included in group A,while 32 patients who performed traditional posterior open transforaminal lumbar interbody fusion(TLIF)were enrolled as group B.The surgical status,rehabilitation process,postoperative lumbar function,pain degree and postoperative complications in the two groups were observed.Result:Operation time,intraoperative blood loss,postoperative blood loss,first postoperative time to the ground and total hospital stay in group A were significantly lower than those in group B,with statistical significance(P<0.05).The scores of visual analogue scale(VAS),Oswestry Disability Index(ODI)and Modified Japanese Orthopaedic Association(M-JOA)of the two groups 1 month and 3 months after surgery were significantly lower than those before surgery,and the scores of group A were significantly lower than those of group B,with statistical significance(P<0.05).The total complication rate of group A was significantly lower than that of group B within 3 months after operation,and the difference was statistically significant(P<0.05).Conclusion:Compared with traditional open surgery,MIS-TLIF for single-segment lumbar degenerative disease can effectively shorten the surgical time,reduce the blood loss,accelerate the rehabilitation process,relieve the pain,and promote the recovery of lumbar function,and it has fewer postoperative complications.
