摘  要:目的了解广西新报告人类免疫缺陷病毒1型(HIV-1)感染者/病人整合酶抑制剂(INIs)耐药流行情况和分子网络特征,为艾滋病的治疗策略和艾滋病防控策略制定提供依据。方法选取2019—2020年在广西壮族自治区疾病预防控制中心VCT门诊新报告的HIV/AIDS病例作为研究对象,并从其血液样本中提取病毒核酸,进行HIV整合酶区基因片段扩增和序列测定,应用美国斯坦福大学的HIV耐药数据库获取耐药信息,并分析基因距离,构建分子网络。结果共收集到283例HIV感染者,获得整合酶区的序列210例,其中流行重组亚型CRF07BC、CRF01AE、CRF08BC和CRF5501B感染者分别占44.76%、32.86%,10.00%和7.62%。共发现2例对整合酶抑制剂耐药的感染者,耐药率0.95%,1例感染CRF5501B毒株发生Q148R主要突变,1例感染CRF07BC毒株发生G163R辅助突变。重组亚型CRF07BC入网率较高。结论广西新报告HIV-1感染者的INIs耐药率处于较低流行水平,加强对风险人群监测和干预。Objective To investigate the drug resistance of human immunodeficiency virus type 1(HIV-1)to integrase inhibitor(INIs)and the molecular network characteristics in newly reported HIV-1 infected patients in Guangxi,China,and to provide scientific reference for the development of treatment strategies and prevention strategies for AIDS.Methods Newly diagnosed HIV-1 infected patients in the VCT clinic of Guangxi Center for Disease Control and Prevention from 2019 to 2020 were selected as the research subjects.The virus nucleic acid was extracted from the blood samples for the amplification and sequencing of HIV integrase gene fragments.The drug resistance information was collected from HIV Drug Resistance Database of Stanford University.The genetic distances were analyzed and the molecular networks were constructed.Results A total of 210 pol sequences of the integrase region were obtained from 283 HIV infected patients.The proportions of CRF07_BC,CRF01_AE,CRF08_BC and CRF55_01B subtypes were 44.76%,32.86%,10.00% and 7.62%,respectively.A total of 2 cases were found to be resistant to INIs,and the resistance rate was 0.95%.One patient was infected by CRF55_01B subtype strain and had a major mutation Q148R while the other one was infected with CRF07_BC subtype strain and had the accessory mutation G163R.Recombinant subtype CRF07_BC had a high rate of network entry.Conclusions The prevalence of INIs resistance among newly reported HIV-1 infected individuals in Guangxi was at a low level.It is necessary to strengthen surveillance and intervention for at-risk populations.
