摘 要:目的:评估650 nm半导体激光鼻内光生物调节治疗(PBM)对高脂血症患者脑灌注的影响。方法:14例高血脂症志愿者加入本次实验,随机分为2组,PBM组和对照组。PBM组每日接受1次鼻腔内激光照射,每次30 min,连续治疗14 d。激光器参数选择波长650 nm,10 Hz脉冲模式,功率密度10 mW/cm^(2)。对照组每日进行1次安慰剂对照治疗,每次30 min,连续治疗14 d,鼻腔内激光照射剂量0 mW/cm^(2)。两组患者均在PBM治疗开始前和全部治疗结束后采集血脂资料。两组患者均在PBM治疗开始前、第一天治疗结束后和全部14 d治疗结束后,接受3次磁共振头颅3DASL(全脑容积灌注动脉自旋标记成像)序列扫描。后处理3DASL序列数据得到脑血流量(CBF)图,测量患者双侧大脑半球半卵圆中心CBF值。血脂参数前后对比采用配对T检验,两组患者三个时间点采集的CBF值对比采用单因素重复测量方差分析及Tukey检验。P<0.05认为具有统计学差异。结果:配对T检验结果显示,PBM组治疗前后总胆固醇(TC),甘油三酸酯(TG),高密度脂蛋白(HDL-c)和低密度脂蛋白(LDL-c)均具有统计学差异(P<0.05),对照组血脂参数治疗前后均无明显差异。方差分析结果显示,PBM组三个时间点采集的CBF值具有明显统计学差异(P<0.05),对照组无明显差异。Tukey检验结果显示,PBM组CBF_(14days)相比CBF_(pre)和CBF_(1day)均有明显变化(P<0.05),CBF_(pre)和CBF_(1day)间无明显变化。结论:重复多次应用650 nm 10 Hz 10 mW/cm^(2)鼻内PBM可有效降低高血脂症患者血脂,调节其全身代谢,提高患者脑血流量,改善脑灌注,预防脑卒中的发生。Objective:To assess the impact of 650 nm semiconductor laser intranasal photobiomodulation therapy on cerebral perfusion in hyperlipidemic patients.Methods:Fourteen hyperlipidemia volunteers were enrolled in this experiment and randomly divided into two groups,PBM group and control group.PBM group received intranasal 650 nm 10 Hz 10 mW/cm^(2)laser treatment 30 min each time once a day,and continuously for 14 days.The control group received placebo-controlled treatment once a day for 30 minutes each time for 14 consecutive days,and intranasal laser irradiation dose was O mW/cm^(2).Blood lipids profiles of two groups were collected pre-and post-14 days PBM treatment.All patients received magnetic resonance wholebrain 3DASL scan pre-PBM,the first day post-and the fourteenth day post-PBM.CBF maps were obtained from processing the 3DASL data,and then the CBF values of bilateral cerebral hemispheres centrum semiovale were measured.Paired T test was employed for the comparison of blood lipids.One-way repeated measures analysis of variance with Tukey post-test were employed for the comparison of CBF values collected at three time points in two groups.P<0.05 was considered statistically significant.Results:Paired T-test results showed that total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDLc)and low density lipoprotein(LDL-c)in PBM group were significantly changed between pre-and post-PBM(P<0.O05),however there was no significant difference in control group.ANOVA results showed that CBF values collected at three time points in PBM group had significant statistical differences(P<0.05),but there was no significant difference in control group.Tukey results showed that CBFi4days of PBM group had significant changes compared with both CBF_(pre) and CBF_(1day)(P<0.05),however CBF_(1day) had no significant change compared with CBF_(pre).Conclusion:Repeated application of 650 nm 10 Hz 10 mW/cm^(2)intranasal PBM can effectively reduce blood lipids in patients with hyperlipidemia,regulate systemic metabolism,improve
