大隐静脉高位结扎、腔内激光闭合术联合血府逐瘀汤治疗大隐静脉曲张患者的疗效及对其术后并发症的影响

摘　　要：目的探讨大隐静脉高位结扎、腔内激光闭合术联合血府逐瘀汤对大隐静脉曲张患者的疗效及对其术后并发症的影响。方法选取2021年1月-2023年3月期间河北工程大学附属医院收治的213例大隐静脉曲张患者,采用随机数字表法分为对照组106例和研究组107例。对照组采用大隐静脉高位结扎、腔内激光闭合术治疗;研究组在对照组基础上采用血府逐瘀汤治疗。治疗3个疗程后,观察比较两组患者临床疗效,术前后凝血指标[凝血酶原时间(Prothrombin time,PT)、活化部分凝血活酶时间(Activated partial thromboplastin time,APTT)、纤维蛋白原(Fibrinogen,FIB)、血小板计数(Platelet,PLT)、红细胞沉降率(Erythrocyte sedimentation rate,ESR)]及炎症因子指标[C-反应蛋白(C-reactive protein,CRP)、降钙素原(Procalcitonin,PCT)、白细胞计数(White blood cell,WBC)、中性粒细胞计数(Neutrophil count,NEUT)]水平、术后并发症及生活质量等。结果治疗后研究组临床总有效率98.13%(105/107)明显高于对照组89.62%(95/106),差异有统计学意义(P<0.05)。治疗后研究组住院时间较对照组明显缩短,差异有统计学意义(P<0.05)。治疗后对照组FIB、PLT、ESR均较治疗前降低(P<0.05),PT、APTT无明显变化(P>0.05);研究组PT、APTT均较治疗前升高(P<0.05),FIB、PLT、ESR均较治疗前降低(P<0.05);且研究组PT、APTT均明显高于对照组(P<0.05),FIB、PLT、ESR均明显低于对照组(P<0.05)。治疗后两组患者CRP、PCT、WBC水平均较治疗前升高,差异有统计学意义(P<0.05),NEUT水平与治疗前比较,差异无统计学意义(P>0.05);且研究组CRP、PCT水平均明显低于对照组,差异有统计学意义(P<0.05),WBC、NEUT水平与对照组比较,差异无统计学意义(P>0.05)。治疗后两组患者生活质量CIVIQ评分均较治疗前升高,差异有统计学意义(P<0.05);且研究组CIVIQ评分较对照组明显升高,差异有统计学意义(P<0.05)。治疗后研究组疼痛、皮肤�Objective To investigate the efficacy of high ligation of the great saphenous vein and intracavitary la⁃ser closure combined with Xuefu Zhuyu Decoction in treating varicose veins of the great saphenous vein and its impact on postoperative complications.Methods A total of 213 patients with great saphenous vein varicosity who were treated at the Affiliated Hospital of Hebei Engineering University from January 2021 to March 2023 were selected and randomly divided into a control group(106 cases)and a study group(107 cases)using a random number table.The control group received high ligation of the great saphenous vein combined with intracavitary laser closure treatment.The study group received the same treatment as the control group,with the addition of Xuefu Zhuyu Decoction.After 3 treatment courses,clinical effica⁃cy,coagulation indicators[prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),platelet count(PLT),erythrocyte sedimentation rate(ESR)],inflammatory markers[C-reactive protein(CRP),procalcitonin(PCT),white blood cell count(WBC),neutrophil count(NEUT)]before and after treatment,postoperative complications,and quality of life were observed and compared between the two groups.Results After treatment,the clinical total effec⁃tive rate in the study group was 98.13%(105/107),significantly higher than 89.62%(95/106)in the control group(P<0.05).The hospitalization time in the study group was significantly shorter than that in the control group(P<0.05).After treatment,the FIB,PLT,and ESR in the control group were significantly lower than those before treatment(P<0.05),but there were no significant changes in PT and APTT(P>0.05).In the study group,PT and APTT increased significantly compared to those before treatment(P<0.05),while FIB,PLT,and ESR were significantly reduced(P<0.05).The study group had significantly higher PT and APTT,and significantly lower FIB,PLT,and ESR than the control group(P<0.05).After treatment,both groups showed significant increases in CRP,PCT,and WBC level
