摘  要:目的探究外周血长链非编码RNA(long non-coding RNA,lncRNA)尿路上皮癌相关基因1(urothelial carcinoma associated1,UCA1)、miR-665表达与急性冠状动脉综合征(acute coronary syndrome,ACS)患者介入治疗后冠状动脉再狭窄发生的相关性。方法选取2022年7月至2023年7月河北大学附属医院心血管内科收治的ACS患者315例,根据冠状动脉再狭窄发生情况分为再狭窄组62例和未再狭窄组253例。采用实时荧光定量聚合酶链反应检测外周血lncRNA UCA1、miR-665表达;采用Pearson分析外周血lncRNA UCA1与miR-665的相关性;采用logistic回归分析影响因素;采用ROC曲线分析外周血lncRNA UCA1、miR-665对ACS患者冠状动脉再狭窄发生的预测价值,并计算曲线下面积(area under curve,AUC)。结果与未再狭窄组比较,再狭窄组外周血lncRNA UCA1表达明显升高,miR-665表达明显降低,差异有统计学意义(1.28±0.22 vs 1.01±0.21,P=0.001;0.76±0.24 vs 1.01±0.22,P=0.001)。Pearson相关性分析显示,ACS患者外周血miR-665与lncRNA UCA1表达呈负相关(r=-0.585,P<0.05)。多因素logistic回归分析显示,lncRNA UCA1是冠状动脉再狭窄发生的危险因素(OR=2.124,95%CI:1.324~3.406,P=0.002),miR-665是冠状动脉再狭窄发生的保护因素(OR=0.765,95%CI:0.653~0.897,P=0.001)。ROC曲线分析显示,外周血lncRNA UCA1、miR-665联合预测冠状动脉再狭窄发生的AUC最高,优于外周血lncRNA UCA1、miR-665单独预测(Z=2.256,P=0.024;Z=2.904,P=0.004)。结论ACS患者介入治疗后外周血lncRNA UCA1和miR-665联合可以更好预测冠状动脉再狭窄发生。Objective To investigate the correlation between the peripheral blood levels of long non-coding RNA urothelial carcinoma associated 1(lncRNA UCA1)and miR-665with the occurrence of coronary restenosis in patients with ACS after interventional treatment.Methods A total of 315ACS patients admitted to the Affiliated Hospital of Hebei University from July 2022to July 2023were recruited and then divided into occurrence group(62cases)and non-occurrence group(253cases)according to the occurrence of coronary restenosis.The expression of lncRNA UCA1and miR-665in peripheral blood samples was detected by real-time fluorescence quantitative PCR.Pearson analysis was applied to analyze the correlation between peripheral blood lncRNA UCA1and miR-665in ACS patients.Logistic regression analysis was used to identify the influencing factors for coronary restenosis in ACS patients.ROC curve was plotted to analyze the predictive value of peripheral blood lncRNA UCA1and miR-665for the restenosis,and their AUC values were calculated.Results The peripheral blood expression of lncRNA UCA1was significantly higher,and that of miR-665was obviously lower in the occurrence group than the non-occurrence group(1.28±0.22 vs 1.01±0.21,P=0.001;0.76±0.24 vs 1.01±0.22,P=0.001).Pearson analysis showed there was a negative correlation between miR-665and lncRNA UCA1 expression levels in the ACS patients(r=-0.585,P<0.05).Multivariate logistic regression analysis indicated that LncRNA UCA1was a risk factor(OR=2.124,95%CI:1.324-3.406,P=0.002),and miR-665was a protective factor(OR=0.765,95%CI:0.653-0.897,P=0.001)for the occurrence of coronary restenosis in ACS patients after interventional treatment.ROC curve analysis revealed that the combination of peripheral blood lncRNA UCA1and miR-665had the highest AUC value in predicting the occurrence of coronary restenosis,which was better than the value of single molecule(Z=2.256,P=0.024;Z=2.904,P=0.004).Conclusion Combination of peripheral blood levels of lncRNA UCA1and.277628 miR-665has good performance for
