摘 要:目的了解杭州地区献血者戊型肝炎病毒感染情况及分布特征。方法收集浙江省血液中心2023年9—11月献血者血液标本5075份,其中ALT正常的献血者标本5037份,ALT升高(>50 U/L)的标本38份。采用酶联免疫吸附法(ELISA)检测抗-HEV IgM、抗-HEV IgG和HEV Ag;采用Fisher检验和χ^(2)检验评估ALT不同水平组的抗-HEV IgM和抗-HEV Ig反应性率的差异;分析献血者人群HEV筛查基本情况分布特征;采用单因素和多因素Logistic回归分析抗-HEV IgM和抗-HEV IgG血清反应性的易感因素;电话随访抗-HEV IgM反应性献血者。结果5075份血液标本的抗-HEV IgM、抗-HEV IgG和HEV Ag反应性率分别为0.45%、22.98%和0%;ALT不同水平组的抗-HEV IgM和抗-HEV IgG反应性率无差异(P>0.05);单因素和多因素分析结果显示,年龄是献血者抗-HEV IgM和抗-HEV IgG反应性的风险因素(P<0.05),不同性别、职业和文化程度的献血者抗-HEV IgM和抗-HEV IgG反应性率无差异(P>0.05)。结论杭州地区存在输血传播HEV(TT-HEV)的潜在风险,需要建立1套经济有效的HEV筛查策略继续对杭州地区献血者定期监测HEV以评估感染风险。Objective To investigate the infection and characteristics of hepatitis E virus among blood donors in Hangzhou.Methods A total of 5075 blood samples of blood donors from Zhejiang Provincial Blood Center from September to November 2023 were collected,including 5037 samples with normal ALT and 38 samples with elevated ALT(>50 U/L).Enzyme-linked immunosorbent assay(ELISA)was used to detect anti-HEV IgM,anti-HEV IgG and HEV-Ag.The Fisher test and Chi-square test were used to evaluate the difference in the reactivity rates of anti-HEV IgM and anti-HEV IgG among different levels of ALT.The distribution characteristics of HEV screening in blood donors were analyzed.Univariate and multivariate logistic regression were used to analyze the susceptibility factors of anti-HEV IgM and anti-HEV IgG seropositivity,and the anti-HEV IgM-reactive blood donors were followed up by telephone.Results The reactivity rates of anti-HEV IgM,anti-HEV IgG and HEV-Ag in 5075 blood samples were 0.45%,22.98%and 0%,respectively.There was no difference in the reactivity rates of anti-HEV IgM and anti-HEV IgG among different levels of ALT(P>0.05),and the results of univariate and multivariate logistic regression analysis showed that age was a risk factor for anti-HEV IgM and anti-HEV IgG reactivity in blood donors(P<0.05),while no difference in the reactivity rates of anti-HEV IgM and anti-HEV IgG among blood donors was noticed in gender,occupation and education level(P>0.05).Conclusion There is a potential risk of transfusion-transmitted HEV(TT-HEV)in Hangzhou,and a cost-effective HEV screening strategy needs to be established to continue regular HEV surveillance in Hangzhou to assess the risk of infection.
