摘 要:目的分析了解漯河市中心医院2019—2023年肺炎链球菌的分布及临床耐药情况,为临床医生提供用药依据。方法回顾性分析2019—2023年漯河市中心医院肺炎链球菌菌株,使用法国梅立埃VITEK2 Compact全自动微生物鉴定药敏仪对采集的细菌进行鉴定和药物敏感性分析,同时采用E-test试验条复核。结果共收集病例2136株,以痰标本为主(97.2%)。科室分布以儿科为主(90.2%),≤5岁儿童占比最多(85.3%),6~18岁人群和成人占比低但有上升趋势。12、1月为肺炎链球菌高发季节,男性感染概率高于女性。药敏实验耐青霉素肺炎链球菌(PRSP)组部分药物耐药率高于PSSP组,5 a内青霉素敏感肺炎链球菌(PSSP)检出率有上升趋势。红霉素、四环素、复方新诺明耐药情况严重;头孢曲松、头孢噻肟、左氧氟沙星、莫西沙星耐药率低。血液分离株PSSP检出率37.1%,痰液分离株PSSP检出率9.9%。结论漯河市中心医院≤5岁儿童感染肺炎链球菌较为严重,抗生素治疗耐药程度不一,临床医师应合理应用抗生素,防止耐药状况加重。Objective To analyze and understand the distribution and clinical drug resistance of Streptococcus pneumoniae in Luohe Central Hospital from 2019 to 2023,and to provide a basis for clinical medication.Methods Retrospective analysis of Streptococcus pneumoniae strains from Luohe Central Hospital from 2019 to 2023,using the French BioMérieux VITEK2 Compact automatic microbial identification and susceptibility analyzer to identify and analyze the drug sensitivity of the collected bacteria,while using E-test strips for verification.Results A total of 2136 cases were collected,mainly sputum samples(97.2%).The department distribution is mainly pediatrics(90.2%),with children under 5 years old accounting for the most(85.3%),and the proportion of people aged 6-18 and adults was low but has an upward trend.December,January were the high incidence seasons for Streptococcus pneumoniae,with a higher infection rate in males than in females.In drug sensitivity experiments,the drug resistance rate of some drugs in the penicillin resistant Streptococcus pneumoniae(PRSP)group was higher than that in the penicillin sensitive Streptococcus pneumoniae(PSSP)group,and the detection rate of PSSP has an upward trend within 5 years.Erythromycin,tetracycline,and third-generation cephalosporin had serious resistance.The resistance rates of ceftriaxone,cefotaxime,levofloxacin,and moxifloxacin were low,with differences between the juvenile and adult groups.The detection rate of PSSP in blood isolates was 37.1%,and the detection rate of PSSP in sputum isolates was 9.9%.Conclusion The infection of Streptococcus pneumoniae in children under 5 years old in Luohe Central Hospital is more serious,and the degree of drug resistance varies with antibiotic treatment.Clinical physicians should use antibiotics reasonably to prevent further deterioration of drug resistance.
