经尿道绿激光汽化术治疗良性前列腺增生的临床效果

摘　　要：目的探究经尿道绿激光汽化术(PVP)治疗良性前列腺增生的临床应用效果。方法回顾性分析河南省直第三人民医院2022年1月至2023年10月收治的良性前列腺增生患者,将行经尿道前列腺等离子电切术(TURP)的患者分为对照组(38例),行PVP治疗的患者分为激光组(50例)。比较两组的临床指征、治疗后的膀胱动力学指标水平、国际前列腺症状评分(IPSS)评分、治疗前后的血清转化生长因子-β1(TGF-β1)、表皮细胞生长因子(EGF)、前列腺素E2(PGE2)水平和并发症发生情况。结果激光组的术后膀胱冲洗时间、尿管留置时间、术后住院时间均短于对照组(P<0.05)。治疗后,激光组的残余尿量(PVR)、IPSS评分水平降幅和最大尿流率(Qmax)水平增幅均高于对照组(P<0.05)。治疗后,两组的TGF-β1、PGE2水平均降低,EGF水平均升高,且激光组的变化幅度大于对照组(P<0.05)。激光组并发症发生率低于对照组(P<0.05)。结论经尿道绿激光汽化术治疗可以改善良性前列腺增生患者的临床指征、膀胱动力学、IPSS评分及血清TGF-β1、EGF、PGE2水平,安全性高,具有较好的临床应用价值。Objective To investigate the clinical effect of photoselective vaporization of the prostate(PVP)in the treatment of benign prostatic hyperplasia.Methods The patients with benign prostatic hyperplasia admitted to the Third People's Hospital of Henan Province from January 2022 to October 2023 were retrospectively analyzed.The patients who underwent transurethral resection of prostate(TURP)were divided into control group(38 cases)and the patients who underwent PVP treatment were divided into laser group(50 cases).The clinical indications,bladder dynamics index level after treatment,international prostatic symptom score(IPSS)score,serum transforming growth faction-β1(TGF-β1),epidermal cell growth factor(EGF),prostaglandin E2(PGE2)levels before and after treatment and the incidence of complications were compared between the two groups.Results The postoperative bladder irrigation time,urinary catheter retention time and postoperative hospital stay in the laser group were lower than those in the control group(P<0.05).After treatment,the decrease of residual urine volume(PVR),IPSS score and the increase of maximum urine flow rate(Qmax)in laser group were higher than those in control group(P<0.05).After treatment,the levels of TGF-β1 and PGE2 in both groups were decreased,and the levels of EGF were increased,and the changes in the laser group were greater than those in the control group(P<0.05).The incidence of complications in laser group was lower than that in control group(P<0.05).Conclusion Transurethral green laser vaporization can significantly improve the clinical indications,bladder dynamics,I-PSS score and serum levels of TGF-β1,EGF and PGE2 in patients with benign prostatic hyperplasia,with high safety and good clinical application value.
