摘  要:目的探究电子胎心监护相关指标联合产妇临床特征对新生儿不良结局的预测价值。方法收集濮阳市妇幼保健院2019年1月至2022年7月产科收治的237例产妇临床资料。观察电子胎心监护相关指标、产妇临床特征等。结果将237例患者按照不同结局分为良好组(165例)与不良组(72例)。两组胎龄、体重指数、新生儿性别、受教育程度、分娩方式比较,差异无统计学意义(P>0.05)。良好组30 min减速区面积高于不良组,年龄低于不良组,妊娠糖尿病、羊水污染、妊娠高血压、胎心率图形异常例数低于不良组(P<0.05)。二元逻辑回归分析显示,结果显示,30 min减速区面积、年龄、妊娠糖尿病、羊水污染、妊娠高血压、胎心率图形异常均是新生儿不良结局的危险因素。电子胎心监护相关指标及产妇临床特征对新生儿不良结局的预测效能较高,曲线下面积(AUC)为0.877。结论电子胎心监护相关指标联合临床特征对于新生儿不良结局有良好的预测价值,可作为临床上预测新生儿不良结局的参照指标。Objective To explore the predictive value of electronic fetal heart monitoring indicators combined with maternal clinical characteristics for adverse outcomes in neonates.Methods Clinical data of 237 cases of parturients admitted to the Department of Obstetrics in Puyang Maternal and Child Health Care Hospital from January 2019 to July 2022 were collected.The related indexes of electronic fetal heart monitoring and clinical characteristics of parturients were observed.Results Two hundred and thirty-seven patients were divided into good group(165 cases)and bad group(72 cases)according to different outcomes.There were no significant differences in gestational age,body mass index,newborn sex,education level and delivery mode between the two groups(P>0.05).The area of 30 min retardation zone in good group was higher than that in bad group,the age was lower than that in bad group,and the number of abnormal cases of gestational diabetes mellitus,amniotic fluid pollution,pregnancy-induced hypertension and fetal heart rate pattern were lower than those in bad group(P<0.05).Binary logistic regression analysis showed that 30 min deceleration zone area,age,gestational diabetes mellitus,amniotic fluid pollution,pregnancy-induced hypertension and fetal heart rate pattern abnormalities were all risk factors for adverse neonatal outcome.Electronic fetal heart monitoring related indexes and maternal clinical characteristics were effective in predicting neonatal adverse outcomes,with an area under curve(AUC)of 0.877.Conclusion Electronic fetal heart monitoring and clinical features have good predictive value for neonatal adverse outcomes,and may be used as a reference index for clinical prediction of neonatal adverse outcomes.
