TIF1-γ抗体阳性皮肌炎合并食管癌1例并文献复习

摘　　要：皮肌炎(DM)是一种特发性炎症性肌病,临床表现为皮疹、对称性近端肌无力、肌酶升高、肌电图示肌源性改变、肌肉活检有特征性改变。不同抗体类型的DM临床表现及预后不同,部分抗体与肿瘤高度相关,比如抗转录中间因子-1γ(TIF1-γ)。本文报道1例中年男性患者因皮疹入院,检查发现TIF1-γ抗体阳性,明确诊断为TIF1-γ阳性DM,胃肠镜检查发现食管癌,转心胸外科手术治疗,术后预后良好,皮疹有所减退。因此,对于TIF1-γ抗体阳性DM患者早期全面筛查肿瘤十分重要,提早发现无症状肿瘤,改善预后。Dermatomyositis(DM)is an idiopathic inflammatory myopathy with clinical manifestations of skin rash,symmetric near-end muscle weakness,elevated muscle enzymes,electromyographic myogenic changes,and characteristic changes in muscle biopsy.Different antibody types of DM have different clinical manifestations and prognosis,and some antibodies are highly correlated with tumor.For example,transcriptional intermediary factor 1-γ(TIF1-γ).In this paper,a middle-aged male patient was admitted to hospital due to rash.TIF1-γantibody was found positive,and the diagnosis was confirmed as TIF1-γpositive DM.Esophageal cancer was found by gastroenteroscopy,and the patient was transferred to cardiothoracic surgery for treatment.Therefore,it is very important for early comprehensive tumor screening in patients with TIF1-γantibody positive DM to detect asymptomatic tumors early and improve prognosis.
