摘　　要：目的:探讨根本原因分析法(RCA)在中成药不合理使用管理中的应用效果。方法:通过我院医院信息管理系统(HIS),提取2022年1~6个月门(急)诊口服中成药处方1200张,参照《处方点评管理办法》《医院处方点评管理规范(试行)》等,进行点评,得出干预措施实施前门(急)诊口服中成药处方不合格率(%)。组建RCA团队,找出导致中成药不合理使用的近端原因,确认根本原因,制定可行性干预措施计划,并执行。干预措施实施后提取1~6个月门(急)诊口服中成药处方1200张进行点评,得出干预措施实施后门(急)诊口服中成药处方不合格率(%)。结果:干预措施实施前门(急)诊口服中成药处方不合格率为20.33%,干预措施实施后门(急)诊口服中成药处方不合格率为4.50%,管控取得了显著成效。结论:中成药不合理使用主要由系统原因造成,RCA有助于发现整个系统管理环节上存在的漏洞,制定可行性计划并执行,从源头上确保中成药的用药安全。Objective To explore the application effect of root cause analysis(RCA)in irrational use management of proprietary Chinese medicine.Methods Through the hospital information management system(HIS)of our hospital,1200 prescriptions of oral Chinese patent medicine for emergency treatment in 1-6 months in 2022 were extracted,and the unqualified rate of oral Chinese patent medicine prescriptions for emergency treatment in Qianmen(Trial)was obtained by referring to the Prescription Review Management Measures and Hospital Prescription Review Management Standards(Trial).Set up an RCA team to find out the proximal cause of the irrational use of proprietary Chinese medicines,identify the root cause,develop a feasible intervention plan,and implement it.After the implementation of intervention measures,1200 prescriptions of oral proprietary Chinese medicine were extracted for 1-6 months af ter the implementation of intervention measures.The unqualified rate of oral proprietary Chinese medicine prescriptions was obtained.Results The unqualified rate of prescription of oral proprietary Chinese medicine was 20.33%when the intervention was implemented,and the unqualified rate of prescription of oral proprietary Chinese medicine was 4.50%when the intervention was implemented,the control has achieved remarkable results.Conclusion The irrational use of proprietary Chinese medicines is mainly caused by system rea sons.RCA helps to find the loopholes in the entire system management process,formulate feasible plans and implement them,and en sure the drug use safety of proprietary Chinese medicines from the source.
