品管圈用于降低非整支剂量药品调配差错率的效果分析

摘　　要：目的分析品管圈(QCC)用于降低静脉用药集中调配中心(PIVAS)非整支剂量药品调配差错率的效果。方法开展品管圈活动,按照计划、执行、检查、处理四个循环步骤(PDCA),利用头脑风暴、帕累托法则、鱼骨图等方法分析非整支剂量药品调配差错率高的真因,根据真因制定解决问题的对策,通过实施对策前后各项数据分析对比品管圈活动对非整支剂量药品调配差错率改善效果。结果通过开展品管圈活动,医院静脉用药集中调配中心非整支剂量药品调配差错率由改善前的10.76%降低到改善后的4.34%,活动目标达成率为105.25%,改善率为59.67%。结论品管圈活动不仅降低了非整支药品调配剂量差错率,提高了工作效率,改善了患者用药安全,而且提高了药师和护士的协作能力和专业知识,保证了药学服务质量。Objective To analyze the effect of quality control circle(QCC)on reducing the dispensing error rate of partial dose drugs in centralized Pharmacy Intravenous Admixture Services(PIVAS).Methods By carrying out quality control circle activities,following the four cycle steps of Planning,Doing,Checking,Acting(PDCA),using brainstorming,the pareto law,fishbone diagram and other methods to analyze the reasons for the high error rate of partial dose drugs dispensing and formulate countermeasures base on the reasons.Through the data analysis before and after the implementation of countermeasures,the improvement effect of quality control circle activities on the dispensing error rate of partial dose drugs was compared.Results The dispensing error rate of partial dose drugs of PIVAS in the hospital decreased from 10.76%(before improvement)to 4.34%(after improvement)through quality control circle activities,the target achievement rate of activities was 105.25%,and the improvement rate was 59.67%.Conclusion Quality control circle activities not only reduce the dosage error rate of partial drug dispensing,improve the work efficiency,improve the drug safety of patients,but also improve the cooperation ability and professional knowledge of pharmacists and nurses,and ensure the quality of pharmaceutical care.
