摘　　要：目的运用网状meta分析的方法评价不同外用制剂预防乳腺肿瘤放疗患者放射性皮炎(radiation dermatitis,RD)的效果。方法计算机检索PubMed、Web of Science、Cochrane Library、Embase、Scopus、中国生物医学文献数据库、中国知网、万方期刊数据库及维普数据库,检索时间为各数据库建库至2024年1月1日,纳入关于不同外用制剂预防乳腺肿瘤放疗患者RD的临床随机对照实验。由2名研究者独立筛选文献、提取资料,使用RoB 2.0进行文献质量评价,采用Stata 14.2软件进行网状meta分析。结果共纳入31篇文献,包括3653例患者,涉及27种干预措施。网状meta分析结果显示:(1)预防RD效果前3为:硼凝胶、凉血解毒膏、洋甘菊。(2)预防2级及以上RD效果前3为:水飞蓟素凝胶、凉血解毒膏、阿尔法软膏。结论硼凝胶在降低乳腺肿瘤放疗患者RD方面总体效果最佳,水飞蓟素凝胶在预防乳腺肿瘤放疗患者2级及以上RD方面总体效果最好,但未来仍需更多高质量随机对照实验来进一步证实。Objective To evaluate the effect of different topical preparations on preventing radiation dermatitis(RD)in patients with breast neoplasms radiotherapy by network meta-analysis.Methods Databases in PubMed,Web of Science,Cochrane Library,Embase,Scopus,China Biomedical Literature Database,China Knowledge Network,Wanfang Data,and VIP Database were searched by computer,and the search time limit was from the establishment of each database to January 1,2024.The randomized controlled clinical trials of different topical preparations for the prevention of radiodermatitis in patients with breast neoplasms radiotherapy were included.The literature was independently screened and the data was extracted by 2 researchers.RoB2.0 was used for literature quality evaluation,and Stata 14.2 software was used for network meta-analysis.Results A total of 31 articles were included,including 3653 patients and involving 27 intervention measures.The results of the network meta-analysis showed that:(1)The top3 in terms of preventing RD are:Boron-based gel,ointment for cooling blood and detoxification,chamomile.(2)The top3 in terms of preventing grade 2 and above RD are:Silymarin gel,ointment for cooling blood and detoxification,alpha ointment.Conclusion The Boron-Based Gel has the best overall effect in reducing RD in breast neoplasms radiotherapy patients,and the Silymarin Gel had the best overall effect on controlling grade 2 and above radiodermatitis.However,more high quality randomized controlled trials are needed to further confirm this point in the future.
