摘 要:目的探讨高频超声在急性踝关节扭伤患者中的应用价值。方法收集2021年1月~2023年10月在我院就诊的95例急性外踝扭伤患者,均进行踝关节高频超声、MRI检查并手术治疗,记录踝关节的损伤情况,对距腓前韧带厚度及损伤情况进行分层诊断,比较高频超声与MRI的诊断效能,分析高频超声诊断与手术结果的一致性。结果(1)急性外踝扭伤患者距腓前韧带的厚度比健侧增加,差异有统计学意义(P<0.05),高频超声与MRI两种检查方法对距腓前韧带厚度的测值差异无统计学意义(P>0.05)。(2)高频超声在撕脱骨折中的检查率高于MRI,差异有统计学意义(P<0.05);普通骨折MRI检查率高于超声,差异有统计学意义(P<0.05);其他韧带、肌腱损伤、关节腔积液情况,两者均无统计学差异(P>0.05)。(3)距腓前韧带损伤的超声分层诊断结果与MRI诊断结果比较,差异无统计学意义(P>0.05)。高频超声诊断结果与手术结果比较,高度一致(Kappa值=0.719,P<0.001)。结论高频超声能够明确诊断踝关节损伤情况,并能够准确的分层诊断,有重要的临床意义。Objective To explore the application value of high-frequency ultrasound in patients with acute ankle sprain.Methods A total of 95 patients with ankle sprains who visited Weifang Hospital of Traditional Chinese Medicine from January 2021 to October 2023 were collected.All patients underwent high-frequency ultrasound and MRI examination of the ankle joint and underwent surgical treatment.The injury of the ankle joint was recorded,and the thickness and injury of the anterior talofibular ligament were stratified for diagnosis of degree Ⅰ,degree Ⅱ,and degree Ⅲ.The diagnostic efficacy of high-frequency ultrasound and MRI was compared,and the consistency between high-frequency ultrasound diagnosis and surgical results was analyzed.Results(1)The thickness of the anterior talofibular ligament in patients with acute lateral ankle sprainwas increased compared to the healthy side,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the measurement of anterior talofibular ligament thickness between high-frequency ultrasound and MRI examination methods(P>0.05).(2)The detection rate of high-frequency ultrasound in avulsion fractures was higher than that of MRI,and the difference was statistically significant(P<0.05);The examination rate of MRI for common fractures was higher than that of ultrasound,and the difference was statistically significant(P<0.05);There was no statistically significant difference(P>0.05)in other ligament and tendon injuries,as well as joint cavity effusion.(3)There was no statistically significant difference(P>0.05)between the ultrasound stratified diagnosis of anterior fibular ligament injury and the MRI diagnosis.The high-frequency ultrasound diagnosis results were highly consistent with the surgical results(Kappa value=0.719,P<0.001).Conclusion High frequency ultrasound can clearly diagnose ankle joint injuries and accurately stratify the diagnosis,which has important clinical significance.
