摘　　要：目的:探究多模式超脉冲点阵CO_(2)激光联合自体脂肪注射对痤疮萎缩性瘢痕的治疗效果。方法:选取2019年12月-2022年12月笔者医院收治的60例痤疮萎缩性瘢痕患者为研究对象,根据住院顺序单双号将患者随机分为对照组和观察组,每组30例。对照组采用超脉冲点阵CO_(2)激光(超脉冲模式)联合自体脂肪注射治疗,观察组进行超脉冲点阵CO_(2)激光(超脉冲模式+Deep FX+Active FX模式)联合自体脂肪注射治疗。比较两组患者治疗7个月后临床疗效、并发症发生情况、瘢痕恢复情况、炎症因子水平及生活质量等。结果:治疗7个月后,观察组治疗总有效率高于对照组(P<0.05);观察组瘢痕面积和红肿面积均小于对照组,温哥华瘢痕量表评分、疼痛评分均低于对照组,整体美学改善量表评分高于对照组,差异均有统计学意义(P<0.05);观察组血清TNF-α、IL-10、IL-7水平均低于对照组(P<0.05);观察组生活质量各维度评分均高于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:多模式超脉冲点阵CO_(2)激光联合自体脂肪注射治疗痤疮萎缩性瘢痕疗效好,安全性高,具有较高的临床推广价值。Objective To explore the therapeutic effects of multiple modes of ultrapulse fractional CO_(2) laser combined with autologous fat injection on atrophic acne scars.Methods 60 patients with atrophic scar in the authou's hospital from December 2019 to December 2022 were selected as the study subjects.Patients were randomly divided into control group(30 cases)and observation group(30 cases)according to the odd-even method.The control group was treated with superpulsed CO_(2) fractional laser(superpulsed model)combined with autologous fat injection,and the observation group was given superpulsed CO_(2) fractional laser(superpulsed model+Deep FX+Active FX model)combined with autologous fat injection.The clinical efficacy,complications,scar recovery,inflammatory factor levels and quality of life were compared between the two groups after 7 months of treatment.Results After 7 months of treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).The scar area and redness and swelling area of the observation group were smaller than those of the control group,the Vancouver scar scale score and pain score were lower than those of the control group,and the overall aesthetic improvement scale score was higher than that of the control group,with statistically significant differences(P<0.05).The serum levels of TNF-α,IL-10 and IL-7 in the observation group were lower than those in the control group(P<0.05).The scores of all dimensions of quality of life in the observation group were higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Multiple modes ultrapulse fractional CO_(2) laser combined with autologous fat injection is effective and safe in the treatment of atrophic acne scars,which has high clinical promotion value.
