纳米微针导入氨甲环酸联合调Q激光治疗黄褐斑对患者皮肤生理指标的影响

摘　　要：目的:探讨纳米微针导入氨甲环酸联合调Q激光对面部黄褐斑患者皮肤生理指标的影响。方法:选取2021年6月-2022年10月笔者医院114例面部黄褐斑患者,随机分为观察组与对照组,每组57例。对照组给予调Q激光治疗,观察组给予纳米微针导入氨甲环酸联合调Q激光治疗,比较两组疗效、黄褐斑面积及严重程度评分(MASI)、皮肤生理参数、VISIA皮肤图像分析仪指标以及不良反应发生情况。结果:观察组治疗有效率78.95%,高于对照组的61.40%(P<0.05);观察组治疗后MASI评分低于对照组(P<0.05);观察组治疗后皮肤黑素指数、经表皮水分丢失量(TEWL)低于对照组,角质层含水量、表皮油脂含量高于对照组(P<0.05);观察组治疗后VISIA皮肤图像分析仪指标斑点、紫外线斑、棕色斑、红色区评分均低于对照组(P<0.05);两组不良反应发生率比较(P>0.05)。结论:纳米微针导入氨甲环酸联合调Q激光治疗面部黄褐斑效果显著,可有效去除色斑,降低皮肤黑素指数,增强皮肤屏障功能,安全性较高。Objective To explore the effects of nanomicroneedle injection of tranexamic acid combined with Q-switched laser on skin physiological indexes in patients with facial melasma.Methods A total of 114 patients with facial melasma in Yan'an Hospital of Traditional Chinese Medicine were enrolled and randomly divided into observation group(57 cases,nanomicroneedle injection of tranexamic acid+Q-switched laser)and control group(57 cases,Q-switched laser)between June 2021 and October 2022.The clinical curative effect,score of melasma area and severity index(MASI),skin physiological parameters,VISIA skin image analyzer indexes and the occurrence of adverse reactions in the two groups were compared.Results The response rate of treatment was higher in observation group than control group(78.95%vs 61.40%,P<0.05).After treatment,MASI score in was lower in observation group than control group(P<0.05).After treatment,skin melanin index and transepidermal water loss(TEWL)were lower in observation group than control group,while cuticle water content and epidermal oil content were higher in observation group(P<0.05).After treatment,scores of VISIA skin image analyzer indexes(spots,ultraviolet spots,brown spots,red areas)were lower in observation group than control group(P<0.05).The incidence of adverse reactions in the two groups was similar(P>0.05).Conclusion The curative effect of nanomicroneedle implantation of tranexamic acid combined with Q-switched laser is significant in patients with facial melasma,which can effectively remove the stain,reduce skin melanin index and enhance skin barrier function,with high safety.
