化痰逐瘀通脉汤在老年急性心力衰竭患者中的应用及对其心肌健康状况的影响

摘　　要：目的研讨化痰逐瘀通脉汤在老年急性心力衰竭(Acute heart failure,AHF)患者中的应用及对其心肌健康状况的影响。方法选取2022年1月至2023年12月经厦门中医院重症医学科确诊为AHF的老年患者80例,以随机数表法确定组别(40例/组),对照组予西医急性心衰基础用药(利尿、扩血管、正性肌力药)治疗,观察组在对照组用药基础上予化痰逐瘀通脉汤温服,比较两组的方案疗效、中医证候得分、心肌健康状况以及不良反应。结果观察组用药后取得的方案总有效率较对照组更高(P<0.05)。用药后,观察组的中医证候(烦躁、气短、倦怠乏力与胸闷)得分相较于对照组均更低(P<0.05)。用药后,观察组的氨基末端B型脑钠肽前体(NTproBNP)、B型脑钠肽(BNP)以及血清肌钙蛋白T(cTnT)测定值相较于对照组均更低(P<0.05)。两组患者间出现的不良反应占比相近(P>0.05)。结论化痰逐瘀通脉汤用于老年AHF患者的治疗效果较理想,能够明显减轻患者的中医证候,改善患者的心肌健康状况,且安全性有保障。Objective To investigate the application of Huatan Zhuyu Tongmai Decoction in elderly patients with acute heart failure(AHF)and its effects on myocardial health.Methods A total of 80 elderly patients diagnosed with AHF by the Intensive Care Unit of Xiamen Traditional Chinese Medicine Hospital between January 2022 and December 2023 were selected for the study.The patients were randomly assigned to two groups(40 patients per group)using a random number table.The control group received conventional Western medicine treatment for acute heart failure(including diuretics,vasodilators,and positive inotropic drugs),while the observation group was additionally treated with Huatan Zhuyu Tongmai Decoction,administered warm.The efficacy of the treatment plans,Traditional Chinese Medicine(TCM)syndrome scores,myocardial health status,and adverse reactions were compared between the two groups.Results The total effective rate of the treatment plan in the observation group was significantly higher than that of the control group(P<0.05).After treatment,the TCM syndrome scores(including irritability,shortness of breath,fatigue,and chest tightness)in the observation group were lower than those in the control group(P<0.05).Moreover,levels of amino-terminal pro B-type natriuretic peptide(NT-proBNP),B-type natriuretic peptide(BNP),and serum cardiac troponin T(cTnT)were significantly lower in the observation group compared to the control group(P<0.05).The incidence of adverse reactions was similar between the two groups(P>0.05).Conclusion Huatan Zhuyu Tongmai Decoction is an effective treatment for elderly patients with AHF,significantly alleviating TCM symptoms and improving myocardial health.Moreover,it demonstrates a favorable safety profile.
