2型糖尿病微血管病变PTX3、VEGF与凝血-纤溶失衡的相关性分析

摘  要:目的:分析2型糖尿病微血管病变患者血清正五聚蛋白3(PTX3)和血管内皮生长因子(VEGF)表达变化与凝血-纤溶失衡的相关性。方法:选取2020年1月—2023年1月信阳市中心医院收治的120例2型糖尿病患者作为研究对象,根据微血管病变情况分为对照组(n=60)和观察组(n=60)。对照组患者无微血管病变情况,观察组患者患有微血管病变。检测患者的PTX3、VEGF及凝血-纤溶指标,分析PTX3及VEGF水平与凝血-纤溶指标的相关性,PTX3、VEGF水平以及凝血-纤溶指标与2型糖尿病微血管病变的相关性。结果:观察组患者的血清PTX3、VEGF水平均明显高于对照组,差异有统计学意义(t=2.826,P<0.05);观察组患者的纤维蛋白原(FIB)、D二聚体(DD)水平明显高于对照组,凝血酶原时间(PT)、凝血活酶时间(APTT)、凝血酶时间(TT)水平明显低于对照组,差异均有统计学意义(t=4.449、7.519、2.267、3.177、5.035,P<0.05);PTX3及VEGF与凝血功能呈正相关(r=0.485、0.421,P<0.05),与纤溶功能呈负相关(r=-0.212、-0.314,P<0.05);PTX3、VEGF是影响2型糖尿病患者发生微血管病变的独立危险因素(OR=1.274、1.473,P<0.05)。结论:2型糖尿病微血管病变患者中PTX3和VEGF表达水平上调,且患者凝血酶原时间缩短、凝血功能增强,纤溶能力减弱。PTX3及VEGF与凝血功能具有相关性,且是影响2型糖尿病患者发生微血管病变的独立危险因素。Objective:In order to explore the correlation between the changes of serum PTX3 and VEGF expression and coagulation-fibrinolysis imbalance in type 2 diabetic patients with microangiopathy.Methods:120 patients with type 2 diabetes admitted to the hospital from January 2020 to January 2023 were selected according to the microangiopathy,and divided into control group(n=60)and observation group(n=60).The control group had microangiopathy,and the observation group had microangiopathy.PTX3,VEGF and coagulation-fibrinolysis indexes of patients were detected,and the correlation between PTX3,VEGF levels and coagulation-fibrinolysis indexes was analyzed.The correlation between PTX3,VEGF,coagulation-fibrinolysis indexes and type 2 diabetic microangiopathy was analyzed.Results:Compared with the control group,the levels of serum PTX3 and VEGF in the observation group were significantly increased(t=2.826,P<0.05).The levels of fibringen(FIB)and D-dimer(DD)in observation group were significantly higher than those in control group,while the levels of prothrombin time(PT),thrombin time(APTT)and thrombin time(TT)in observation group were significantly lower than those in control group,with statistically significant difference(t=4.449,7.519,2.267,3.177,5.035;P<0.05).PTX3 and VEGF were positively correlated with coagulation function(r=0.485,0.421,P<0.05),and negatively correlated with fibrinolytic function(r=-0.212,-0.314,P<0.05).PTX3 and VEGF were independent risk factors for microangiopaopathy in type 2 diabetes mellitus(OR=1.274,1.473;P<0.05).Conclusion:The expression levels of PTX3 and VEGF are up-regulated in type 2 diabetic microangiopathy patients,the prothrombin time is shortened,the coagulation function is enhanced,and the fibrinolytic ability is weakened.PTX3 and VEGF are correlated with coagulation function,and are independent risk factors for microangiopathy in patients with type 2 diabetes mellitus.
