摘　　要：选择合适的压力性损伤风险评估工具,尽早识别高危因素,是预防压力性损伤的关键环节。聚焦《预防ICU成人患者压力性损伤过程质控工具包》评估措施,阐释压力性损伤的评估时机与内容、风险评估工具及特点,并从量表选择、护士培训、信息化建设等方面提出了评估工具使用建议,以引导护士准确把握评估时机,选择合适的评估工具。Selecting appropriate risk assessment tool and identifying high-risk factors of pressure injury as early as possible are the key links to prevent pressure injury.Focusing on the assessment measures of"Quality Control Toolkit for Preventing of Pressure Injury in Adult Patients in ICU",this paper explains the timing and content of pressure injury assessment,risk assessment tool and characteristics of pressure injury.It put forward suggestions on the use of assessment tool from the aspects of scale selection,nurse training and information construction,so as to guide nurses to accurately grasp the timing of assessment and select appropriate assessment tool.
