摘　　要：目的:探讨性别差异对急性A型主动脉夹层(acute type A aortic dissection,ATAAD)患者术后住院期间(手术后30d)死亡的影响。方法:回顾性分析2015年11月至2020年12月于首都医科大学附属北京安贞医院心脏外科因ATAAD接受急诊手术的432例患者(年龄> 18岁)的临床资料。根据性别将患者分为男性组和女性组。采用单因素和多因素Logistic回归分析研究性别与术后30天内死亡的关系。结果:纳入的患者中,男性340例(78.7%),平均年龄为(46.4±10.6)岁;女性92例(21.3%),平均年龄为(51.5±11.7)岁。术后30天内,共45例(10.4%)患者死亡,其中女性组死亡15例(16.3%),男性组死亡30例(8.8%),两组间差异有统计学意义(P=0.037)。多因素Logistic回归分析显示性别与ATAAD患者术后30天内死亡无相关性(OR=0.792,95%CI:0.311~2.014,P=0.624)。结论:在ATAAD患者中,性别差异对手术后30天内的死亡没有影响。Objective:To investigate the effect of gender differences on in-hospital mortality(within 30 days after surgery)after emergency surgery for acute type A aortic dissection(ATAAD).Methods:The clinical data of 432 patients(aged>18)who underwent emergency surgery for ATAAD in the Department of Cardiac Surgery,Beijing Anzhen Hospital Affiliated to Capital Medical University from November 2015 to December 2020 were retrospectively analyzed.Patients were divided into male and female groups based on gender.Univariate and multivariate Logistic regression analysis was used to study the relationship between sex and mortality within 30 days after surgery.Results:Among the included patients,340(78.7%)were males,with a mean age of(46.4±10.6)years;92(21.3%)were females,with a mean age of(51.5±11.7)years.Within 30 days after surgery,a total of 45(10.4%)patients died,including 15 females and 30 males,with a statistically significant difference between the female and male groups(P=0.037).Multivariate Logistic regression analysis showed no correlation between gender and mortality within 30 days after surgery in ATAAD patients(OR=0.792,心肺血管病杂志2025年1月第44卷第1期Journal of Cardiovascular&Pulmonary Diseases,January 2025,Vol.44,No.195%CI:0.311-2.014,P=0.624).Conclusions:In patients with ATAAD,gender differences have no effect on postoperative mortality within 30 days.
