摘 要:目的:研究参乌益肾方对慢性肾脏病(CKD)2~3期患者肾功能保护和对白介素-4(IL-4)、白介素-10(IL-10)、干扰素-γ(INF-γ)的影响。方法:选择53例CKD2~3期且中医证型为气阴两虚证的患者给予参乌益肾方治疗,观察他们治疗前后的肾功能、24h尿蛋白定量(24hUpro)、中医症状积分,INF-γ、IL-4、IL-10、INF-γ/IL-4、INF-γ/IL-10水平等指标的变化。结果:与健康人群相比,CKD 2~3期患者INF-γ水平上升,IL-4、IL-10水平下降,INF-γ/IL-4、INF-γ/IL-10上升(P<0.05)。治疗3月后,治疗组中医症状积分和血尿素(BUN)、肌酐(Scr)、24hUpro、INF-γ、INF-γ/IL-4、INF-γ/IL-10水平较治疗前下降(P<0.05),而IL-4、IL-10水平上升(P<0.05)。结论:参乌益肾方可改善CKD 2~3期患者的肾功能,通过改善INF-γ、IL-4、IL-10、INF-γ/IL-4、INF-γ/IL-10水平而调节Th1、Th2可能是其作用机制。Objective:To study the effect of Shenwu Yishen decoction on renal function protection and influence of INF-γ,IL-4,IL-10 in patients with chronic kidney disease(CKD)at stage 2-3.Methods:A total of 53 patients with CKD at stage and 2-3 and TCM syndrome type of Qi Yin deficiency were treated with Shenwu Yishen decoction and the changes in renal function,24-hour urinary protein quantification(24hUpro),TCM syndrome scores,INF-γ,IL-4,IL-10,INF-γ/IL-4,INF-γ/IL-10 levels and other indicators before and after treatment were observed.Results:Compared with the healthy population,INF-γlevel and INF-γ/IL-4,INF-γ/IL-10 were higher and IL-4 and IL-10 levels were lower in the patients(P<0.05),TCM symptom score,blood urea(BUN),creatinine(Scr),24hUpro,INF-γlevels,INF-γ/IL-4 and INF-γ/IL-10 in the treatment group decreased,while IL-4 and IL-10 levels increased after 3 months of treatment than before(P<0.05).Conclusion:Shenwu Yishen decoction can improve the renal function of patients with CKD at stage 2-3,and the regulation of Th1 and Th2 by improving the levels of INF-γ,IL-4,IL-10,IFN-γ/IL-4 and IFN-γ/IL-10 may be the mechanism of its action.
