四妙排石汤联合快速康复外科在输尿管结石微创治疗的临床应用研究

摘　　要：目的:探究四妙排石汤联合快速康复外科(FTS)在输尿管结石(UC)微创治疗的临床应用价值。方法:选取UC患者90例,随机分为对照组和试验组,各45例。对照组患者于微创治疗围手术期进行常规干预;试验组于微创治疗围手术期介入FTS理念,术后联合四妙排石汤治疗。比较两组碎石成功率、2次碎石率、手术及恢复指标(手术时间、术中出血量、术后住院时间、住院费用)、术中及术后并发症发生率;并比较两组术后3 d、1周、2周、1个月时的清石率。结果:与对照组比,试验组碎石成功率更高,2次碎石率更低;术后住院时间更短、住院费用更低、术中出血量更少;术后3 d、1周、2周、1个月时的清石率高于对照组,并发症发生率低于对照组(P<0.05)。结论:四妙排石汤联合FTS应用于UC微创治疗中,能够提高碎石成功率、清石率,降低并发症发生率,缩短住院时间,减少住院费用。Objective:To evaluate the clinical effectiveness of Simiao Paishi decoction combined with fast-track surgery(FTS)in the minimally invasive treatment of ureteral calculus(UC).Methods:Ninety UC patients were randomly divided into a control group and an experimental group,with 45 patients each.The control group received routine perioperative interventions during minimally invasive treatment,while the experimental group underwent FTS intervention during the perioperative period and was treated with Simiao Paishi decoction postoperatively.The lithotripsy success rate,second lithotripsy rate,and various surgical and recovery indicators(operation time,intraoperative blood loss,postoperative hospital stay,hospitalization costs),and the incidence of intraoperative and postoperative complications were compared between the two groups.Additionally,the stone clearance rates at 3 days,1 week,2 weeks,and 1 month after surgery were also compared.Results:Compared with the control group,the experimental group had a higher lithotripsy success rate and a lower second lithotripsy rate,a shorter postoperative hospital stay,lower hospitalization costs,and less intraoperative blood loss.Additionally,the stone clearance rates at 3 days,1 week,2 weeks,and 1 month after surgery were higher,and the incidence of complications was lower in the experimental group(P<0.05).Conclusion:The combination of Simiao Paishi decoction and FTS in the minimally invasive treatment of UC can significantly improve the lithotripsy success rate,enhance the stone clearance rate,reduce the incidence of complications and hospitalization costs and shorten the hospital stay.
