基于“心与小肠相表里”观察黄连阿胶汤加减辨治炎症性肠病伴失眠的效果

摘　　要：目的:观察黄连阿胶汤加减辨治炎症性肠病伴失眠的效果。方法:将60例炎症性肠病伴失眠患者随机分为对照组和观察组,各30例。对照组给予常规西药治疗,观察组在对照组基础上给予黄连阿胶汤加减方口服,连续治疗8周。对比两组的总有效率、改良Mayo评分、匹兹堡睡眠质量指数量表(PSQI)评分、肠道菌群和血清中相关因子水平。结果:观察组的有效率和双歧杆菌、乳杆菌、血清中5-羟色胺、胰岛素样生长因子-1、转化生长因子-β_(1)水平高于对照组;改良Mayo、PSQI评分和大肠杆菌、白介素-21水平低于对照组(P<0.05)。结论:黄连阿胶汤加减可有效改善炎症性肠病伴失眠患者的临床症状,提高睡眠质量,改善肠道菌群。Objective:To observe the effect of modified Huanglian Ejiao decoction in the treatment of inflammatory bowel disease accompanied by insomnia.Methods:Sixty patients with inflammatory bowel disease and insomnia were randomly divided into a control group and an observation group,with 30 patients each.Both groups received conventional Western medicine treatment,while the observation group was additionally given modified Huanglian Ejiao decoction for oral administration over 8 consecutive weeks.The total effective rate,modified Mayo score,Pittsburgh sleep quality index(PSQI)score,intestinal microbiota,and serum levels of related factors were compared between the two groups.Results:The effective rate,levels of Bifidobacterium,Lactobacillus,5-hydroxytryptamine,insulin-like growth factor 1,and transforming growth factor β_(1) were higher,and the modified Mayo score,PSQI score,levels of Escherichia coli,and interleukin 21 were lower in the observation group compared to the control group(P<0.05).Conclusion:Modified Huanglian Ejiao decoction can effectively improve clinical symptoms,enhance sleep quality,and modulate intestinal microbiota in patients with inflammatory bowel disease accompanied by insomnia.
