高温高湿环境下兔软组织爆炸伤后感染动物模型建立的研究

摘  要:目的:建立高温高湿环境下兔软组织爆炸伤后感染模型,为研究高温高湿环境下此类创伤后感染治疗提供依据。方法:中国人民解放军南部战区总医院于2023年6月~2023年7月选取24只成年新西兰大白兔进行实验,随机分为常温常湿组(对照组)与高温高湿组(研究组)各12只。全身麻醉备皮后分别在不同温度和湿度条件下距兔左侧胸部1 cm处放置电雷管并爆炸致伤。在不同时间点测量创面面积,苏木精-伊红(HE)染色评估创面周围软组织炎性反应及感染情况。收集创面渗液,评估细菌负荷及感染严重程度。结果:爆炸后12 h研究组创伤面积大于对照组,差异有统计学意义(t=7.012,P<0.05)。两组HE染色均可见炎性反应浸润及组织坏死。研究组在创伤后12 h即出现明显组织坏死及感染,而对照组在创伤后24 h才出现组织腐烂和感染。高温高湿环境下,创面面积和深度越大,细菌负荷越重。结论:高温高湿条件下兔软组织爆炸伤后感染模型能真实模拟特殊条件下爆炸伤后软组织感染情况,具有良好的重复性与稳定性,可用于高温高湿条件下软组织爆炸伤后感染理论和治疗的实验研究。Objective To establish an animal model of infection after rabbit soft tissue explosion under high temperature and high humidity environment,and to provide a basis for studying the treatment of such post-traumatic infections in high temperature and high humidity environments.Method From June to July 2023,24 adult New Zealand white rabbits were selected and divided into normal temperature and normal wet group(control group)and high temperature and high humidity group(study group)by using random number table method in the southern theater General Hospital of Guangzhou People's Liberation Army,with 12 animals in each group.After general anesthesia,an electrodetonator was placed 1 cm away from the left chest of the rabbit under different temperature and humidity conditions and exploded,causing injury.The wound area was measured at different time points,and HE staining assessed the inflammation and infection of the soft tissue around the wound.The wound exudate was collected to assess the bacterial load and the severity of the infection.Results The trauma area of the study group at 12 hours after the explosion was greater than that of the control group,the difference was statistically significant(t=7.012,P<0.05).Inflammatory infiltration and tissue necrosis were observed by soft tissue HE staining in both groups.Significant tissue necrosis and infection occurred only 12 hours after trauma,while the control group showed tissue decay and infection only 24 hours after trauma.Under high temperature and humidity environment,the larger the wound area and depth,the heavier the bacterial load.Conclusion The rabbit soft tissue explosion infection model under high temperature and high humidity conditions can truly simulate the soft tissue infection situation after explosion injury under special conditions,with good repeatability and stability,and can be used for experimental research on the theory and treatment of soft tissue explosion infection under high temperature and high humidity conditions.
