摘　　要：目的:探讨瑞芬太尼联合环泊酚对腹腔镜子宫肌瘤剔除术患者苏醒质量及认知功能的影响。方法:选取盐城市第一人民医院2021年6月~2023年6月行腹腔镜子宫肌瘤剔除术的78例患者,按随机数字表法分为对照组和观察组,各39例。对照组使用瑞芬太尼联合丙泊酚进行麻醉,观察组使用瑞芬太尼联合环泊酚进行麻醉,观察对两组患者麻醉后认知功能、苏醒质量、不良反应发生率、疼痛评分和炎性反应的影响。结果:观察组恢复自主呼吸时间、苏醒时间、恢复定向力时间以及拔管时间均短于对照组,麻醉后的疼痛视觉模拟评分(VAS)和炎性因子水平均低于对照组,差异有统计学意义(P<0.05)。观察组麻醉24 h后的认知水平高于对照组,且不良反应发生率更低,差异有统计学意义(P<0.05)。结论:对腹腔镜子宫肌瘤剔除术患者给予瑞芬太尼联合环泊酚进行麻醉能够有效提高患者的苏醒质量和认知功能,有效减轻患者的炎性反应和疼痛程度,并降低不良反应发生率。Objective To investigate the effect of remifentanil combined with cyclopofol on the quality of recovery and cognitive function of patients undergoing laparoscopic myomectomy.Method A total of 78 patients who underwent laparoscopic myomectomy from June 2021 to June 2023 in our hospital were selected and divided into control group and observation group according to the random number table method,with 39 cases in each group.The control group was anesthetized with remifentanil combined with propofol,and the observation group was anesthetized with remifentanil combined with cyclopofol,to observe the effects on cognitive function,wake quality,incidence of adverse reactions,pain score and inflammatory response after anesthesia in the two groups.Results The time to recover spontaneous breathing,the time to wake up,the time to regain orientation and the time to extubation in the observation group were shorter than those in the control group,and the VAS score and inflammatory factor levels after anesthesia were lower than those in the control group(P<0.05).The cognitive level of the observation group was higher than that of the control group after 24 hours of anesthesia,and the incidence of adverse reactions was lower(P<0.05).Conclusion Anesthesia with remifentanil combined with cyclopofol can effectively improve the quality of awakening and cognitive function,reduce the inflammatory response and pain of patients,and reduce the incidence of adverse reactions.
