清代著名医家王旭高从肝论治痰证学术经验探讨

摘　　要：痰浊可导致多种疾病的发生,是体内水液代谢障碍的病理产物。历代医家多从肺脾肾三脏论治痰证,而从肝论治者少。王旭高(1798—1862),名泰林,江苏无锡人,被后人尊称为“治肝楷模”。观其医案,王氏治疗痰证有从肝论治者,其理、法、方、药清晰巧妙,疗效显著。作者结合《王旭高医案》痰证医案及《西溪书屋夜话录》治肝三十法,总结出王旭高从肝治疗痰证经验,分别是疏肝理气化痰、疏肝健脾化痰、清金平肝化痰;对于肝风痰火胶结为病者,多以滋阴熄风化痰并进,若肝风痰火上盛,又当清肝泻火开痰为先。Phlegm can lead to many diseases and is the pathological product of water metabolism disorder in the body.Phlegm syndrome is often treated from the perspective of the lung,spleen and kidney,but few from the perspective of the liver.WANG Xugao(1798-1862),whose real name was Tailin,was from Wuxi,Jiangsu Province.He was honored by later generations as a“model for liver treatment”.Looking at his medical records,Wang’s treatment of phlegm diseases is based on the liver.His theory,method,prescription and medicine are clear and ingenious,and the effect is remarkable.Combining the phlegm syndrome medical records in“Wang Xugao’s Medical Records”and the thirty methods of liver treatment in“Xixi Book House Night Talk Records”,the author summarizes WANG Xugao’s experience in treating phlegm syndrome from the liver,which are soothing the liver,regulating qi and resolving phlegm,soothing the liver,strengthening the spleen and resolving phlegm,and clearing the liver and calming the liver to resolve phlegm;for patients with liver wind,phlegm and fire,nourishing yin,extinguishing wind and resolving phlegm are often used simultaneously.If the liver wind,phlegm and fire are strong,clearing the liver,purging fire and resolving phlegm should be the first step.
