摘　　要：不稳定型心绞痛(UA)作为冠心病的一种类型,介于稳定性心绞痛及急性心梗之间,不加以控制极易引发各类心血管事件发生。传统医学中据其表现将其归于“胸痹”“心痛”等范畴,普遍认为其病机归于“血瘀”,合并痰浊、寒凝、气滞等,张叶祥教授认为肾为先天之本,元气之根,肾脏亏虚,心肾失交,脉气不充,血脉滞涩,形成血瘀,瘀久酿毒,毒伤血脉,总结UA根本病机为“肾虚血瘀,因瘀致变”,提出“补肾”“活血”“解毒”治疗三法。Unstable Angina(UA),as a type of coronary heart disease,lies between stable angina and acute myocardial infarction.If left uncontrolled,it can easily lead to various cardiovascularevents.In traditional medicine,it is categorized under terms such as"chest obstruction"and"heart pain,"with the general belief that its pathogenesis is related to"blood stasis,"often combined with phlegm turbidity,cold obstruction,and qi stagnation.Professor Zhang Yexiang believes that the kidney,as the foundation of congenital essence and the root of vital energy,plays a cru cial role.Kidney deficiency leads to a lack of heart-kidney coordination,insufficient vital energy,and stagnant blood flow,resulting in blood stasis.Over time,blood stasis transforms into toxicity,which damages the blood vessels.The fundamental pathogenesis of UA is summarized as"kidney deficiency and blood stasis,with stasis leading to transformation."He proposes a three-pronged treatmen t approach:"nourishing the kidney,""invigorating the blood,"and"detoxifying."
