王朝亮“心肾同治”法治疗慢性心衰合并不寐的临床经验

摘　　要：慢性心力衰竭简称慢性心衰致,病因素繁多,以心脏的结构和功能受损致使心脏的充盈和收缩出现不同程度的受限,心排血量降低为主要临床特征。临床上常是各种心血管疾病的终端事件。多数慢性心衰患者就诊时询问病史,恢复过程中常伴随失眠症状,所选西药治疗会因患者肝肾功能、电解质、既往病情等方面使用受限,无法因人因证施治。王朝亮主任医师认为,慢性心衰合并不寐病位责之于心肾,以心肾亏虚,心肾不交为主要病因,辨证治法更应心肾同治,调和阴阳。桂枝甘草龙骨牡蛎汤加减在临床辨治过程中温肾阳、益心气、交通心肾对慢性心衰合并不寐有确切的疗效。Chronic heart failure(CHF)has many pathogenic factors,and the main clinical features are the damage of the structure and function of the heart,which leads to different degrees of limitation of the filling and contraction of the heart and the reduction of cardiac output.It is often the terminal event of various cardiovascular diseases in clinic.Most patients with CHF are asked about their medical history when they visit the doctor.Insomnia is often accompanied by symptoms during the recovery process.The selected western medicine treatment is limited by the patient’s liver and kidney function,electrolytes,previous medical conditions,etc.,and it is impossible to treat it according to the individual and the syndrome.Chief Physician WANG Chaoliang believes that the location of chronic heart failure combined with insomnia is the heart and kidney,with heart and kidney deficiency and heart and kidney disharmony as the main causes.The syndrome differentiation and treatment method should be to treat the heart and kidney together and harmonize yin and yang.In the process of clinical differentiation and treatment,Guizhi Gancao Longgu Muli Decoction warms the kidney yang,benefits the heart qi,and connects the heart and kidney,which has a definite effect on chronic heart failure combined with insomnia.
